摘要
目的 研究妊高征胎儿下腔静脉直径和血流流速曲线的超声检测与意义。方法 90例孕妇(孕龄28周~42周)行常规超声检查后,对胎儿下腔静脉行二维及多普勒超声检查。实验分为两组:正常对照组50例,PIH组40例,皆按孕周再分2亚组(28~36周组与36~42周组)。结果 胎儿下腔静脉多普勒血流流速曲线图为三相型:大“v”、小“e”波和反向“a”波。对照组下腔静脉的血流参数中,Vmax、Vmin、TAV、S/D、A/S在妊娠中期与晚期之间无显著差异,而下腔静脉直径(D)在妊娠晚期较中期明显增加(P<0.05)。TAV在PIH组显著低于对照组,尤以妊娠晚期为著(P<0.05)。S/D、A/S在妊娠中期HH组高于对照组,但无统计学差异;在妊娠晚期PIH组显著高于对照组(P<0.05)。下腔静脉直径(D)在PIH组显著高于对照组,尤以妊娠晚期为著(P<0.05)。结论 多普勒超声探测胎儿下腔静脉血流流速曲线,对PIH时胎儿心功能评价及围产期并发症的预测具有一定意义。
Objective To study the methods and value of detecting the diameter and flow velocity tracing of fetal inferior vena cava(IVC) by ultrasonography in pregnancy - induced hypertension. Methods Ninty pregnant women with gestational age of 28-42 weeks were examined by conventional ultrasonography, then the IVC were investigated by two dimensional and duplex Doppler ultrasonography. These were classified into two groups: control group(50 cases) and PIH group(40 cases), which were sub - divided into two subgroups respectively(28-36 week group and 36 -42 week goup). Results The Doppler flow velocity tracing of fetal IVC consist of three phases: big “v”, small “e” and retrograde “a” . In the control group, there were no significant difference in Vmax,Vmin,TAV, S/D and A/S of IVC during medial and late gestational age; whereas the diameter of IVC was significantly less in medial gestational age than that in late gestational age(P< 0.05) . TAV in PIH group was significantly lower than that in the control group, especially in the late gestational age(P < 0.05) . S/D and A/S in PIH group were higher than that in the control group without statistical difference in medial gestational age; there was a significant difference in the late gestational age(P< 0.05) . The diameter of IVC in PIH group was significantly larger than that in the control group, especially in the late gestational age(P< 0.05) . Conclusion It is useful to investigate the fetal IVC flow by duplex Doppler ultrasonography in the assessment of fetal heart function and prediction of complications during perinatal period in PIH women.
