肝硬化Child分级与骨转换指标的临床研究

摘要 目的 研究肝硬化Child分级与骨转换指标的关系。方法 测定33例肝硬化患者(男23例,女10例)和30例健康人群(男19例,女11例)的血清钙(Ca2+)、磷(PO3-)、碱性磷酸酶(AKP)、尿钙/肌酐(Ca2+/Cr)以及尿脱氧吡啶啉(DPD)的水平,对检测结果进行分析。结果 肝硬化患者随着肝损害程度的加重,血清Ca2+、PO3-水平逐渐降低,而血清AKP、尿Ca2+/Cr以及尿DPD水平则逐渐升高。其中以尿DPD的变化更为显著。结论 肝硬化患者随着病情的进展,呈现出骨形成降低,骨吸收增强的特征;尿DPD是反映骨吸收的一项敏感而特异的指标,动态监测骨转换指标特别是尿DPD的变化,对肝硬化病情的评估具有重要的意义。 Objective To study the relationship of child-grade in patients with liver cirrhosis and the biochemical markers of bone turnover. Methods Thirty-three patients with liver cirrhosis(23 males and 10 females)and thirty healthy persons (19 males and 11 females)were respectively selected for analyzing the levels of serum calcium (Ca2+), phosphate (PO-3), total serum alkaline phosphatase(AKP),urinary calcium/creatinine ratio(Ca2+/Cr) and urinary deoxypyridinoline(DPD). Results With the further development of liver impairment the concentration of serum calcium and phosphate in the patients with liver cirrhosis was gradually decreasing,while the levels of serum AKP,urinary Ca2+/Cr and urinary DPD in the patients were gradually increasing,and the level of urinary DPD changed more significantly among them. Conclusion With the further development of liver impairment,reduced bone formation and increased bone resorption occurs in patients with liver cirrhosis;Urinary DPD is a sensitive and specific biochemical marker of bone absorption. Our findings suggest that the analysis of the biochemical markers of bone turnover,especially the urinary DPD,may provide some important reference value for the condition of liver cirrhosis.
