成人颅内感染血清心肌酶和脑脊液乳酸脱氢酶的改变及其临床意义 被引量:15

摘要 目的 观察成人颅内感染急性期血清心肌酶和脑脊液 (CSF)乳酸脱氢酶 (LDH)的改变及其临床意义。方法 用酶速率法测定颅内感染患者 (发病 7天内 )血清心肌酶 [天门冬氨酸氨基转移酶 (AST)、LDH、α 羟丁酸脱氢酶 (α HBDH)、肌酸激酶 (CK)、肌酸激酶心型 (CK MB) ],并行腰穿取CSF检测LDH、白细胞数、蛋白定量等。结果 颅内感染组 [病毒性脑炎 (病脑 ) 31例 ,结核性脑膜炎 (结脑 ) 11例 ,其它颅内感染 4例 ]血清心肌酶及CSFLDH均显著高于对照组 (P <0 0 5~ 0 0 1) ;病脑组与结脑组的血清心肌酶比较无显著差异 (P >0 0 5 ) ;结脑组CSFLDH明显高于病脑组 (P <0 0 1) ;CSF中LDH与血清LDH、CSF白细胞数和蛋白均无相关性(r=0 16 9,P >0 2 0 ;r=0 2 83,P >0 0 5 ;r=0 2 76 ,P >0 0 5 )。结论 成人颅内感染急性期存在明显的血清心肌酶及CSFLDH增高 。 Objective To observe changes and clinical significance of serum myocardial enzyme and cerebrospinal fluid (CSF) LDH in adults with acute intracranial infection Methods To estimate myocardial enzyme level (AST, LDH, α HBDH, CK and CK MB) in serum of the adult patients with acute intracranial infection in 7days by rate method, and to measure LDH level, leukocyte count and protein quantitation in CSF at the same time Results The serum myocardial enzyme and CSF LDH level of intracranial infection group(31 cases with viral encephalitis, 11 cases with tuberculous meningitis and 4 cases with other intracranial infection) were significantly higher than those of normal control group ( P < 0 05~ P < 0 01) There were no significant differences in serum myocardial enzyme level between viral encephalitis group and tuberculous meningitis group ( P > 0 05) The CSF LDH level of tuberculous meningitis group was prominently higher than those of viral encephalitis group ( P < 0 01) No correlations were found in CSF LDH level with serum myocardial enzyme level( r =0 169, P > 0 20), with CSF leukocyte count( r =0 283, P > 0 05) and CSF protein quantity ( r =0 276, P > 0 05) Conclusion There is significant increase in the levels of serum myocardial enzyme and CSF LDH of adult patients with acute intracranial infection The levels of serum myocardial enzyme and CSF LDH is helpful to the diagnosis and the differential diagnosis of intracranial infection in early stage
