摘要
目的 总结巨大左心室 [舒张末期直径 (EDD) >70 m m ]瓣膜病人外科治疗经验 ,提高其手术疗效。方法 对 34例巨大左心室心脏瓣膜病患者实行瓣膜替换术。术前平均 EDD 81.0 mm ,收缩末期直径 (ESD)6 1.0 m m ,射血分数 (EF) 0 .4 3,射血分数缩短 (FS) 0 .2 1;心功能 级 11例 , 级 2 3级。主动脉瓣替换术 (AVR) 8例 ,二尖瓣替换术 (MV R) 9例 ,AVR+MVR17例。结果 本组治愈出院 32例。围手术期及术后早期死亡 2例 ,晚期死亡 2例。结论 强调温血灌注改进心肌保护技术 ,保留二尖瓣装置和加强围手术期处理 。
Objective In order to improve the results of valve replacement operation for the patients with giant left ventricle (EDD>70 mm),a retrospective study was carried out reviewing our experience.Methods Thirty-four patients with giant left ventricle underwent valve replacement operation,AVR in 8,MVR in 9 and AVR+MVR in 17.Preoperatively LVEDD was 80.1 (70～90)mm,LVESD 60.1 (40～79)mm,LVEF 0.43 (0.29～0.50) and FS 0.21 (0.14～0.25).Heart function (NYHA) was class Ⅲ in 11 and class Ⅳ in 23.Results Thirty-two long-term survivors were followed up for 6 months to 7 years,heart function was improved to class Ⅰ in 21 patients,class Ⅱ in 8 and class Ⅲ in 1;early death occurred in 2 patients,late death occurred in 2 patients.Conclusion The study suggests that continuing warm blood cardial perfusion (CWBCP)can significantly improve the results of valve replacement operation for the patients with giant left ventricle.
