摘要
目的 :探讨家族遗传性左室室壁瘤的发病特点及MRI诊断价值。方法 :搜集有明显家族聚集性的一家系 11人的健康状况资料 ,对现存的 6人中经心电图和超声心动图确诊的 4人 ,采用MR“黑血”和“白血”技术进行扫描检查 ,测量左室室壁瘤累及的范围、厚度 ,计算出瘤壁缩短增厚率和瘤体占左心室容积的百分比值 ,分析瘤壁运动状态、信号强度及附壁血栓。结果 :本组病例室壁瘤累及左心室 13个节段。其中心尖部 2例 ,心尖部、前壁、侧壁 1例 ,心尖部、前壁、侧壁、下壁和室间隔 1例。收缩期室壁瘤室壁厚度最大 4mm ,最小 1mm ,平均 2 .3mm。舒张期室壁瘤室壁厚度最大 3.5mm ,最小 1mm ,平均 2 .2mm。增厚率最大为 12 .5 %,最小为 0。瘤壁心肌信号强度正常 1例 ,下降 3例。占左心室容积最大为 84 .5 %,最小为 8.8%。室壁瘤室壁运动降低 2例 ,运动消失 1例 ,反向 (矛盾 )运动1例。 4例中仅有 1例见附壁血栓。结论 :家族遗传性左室室壁瘤有一定的特殊性 ,MRI所见能反映其病理变化 ,优于其他医学影像学方法。
Objective: To evaluate the characteristic findings of familial and hereditary ventricular aneurysm (VA) of the left ventricle(LV) and the value of MR diagnosis.Methods: To gather data of physical conditions of 11 persons in a family that was clearly familial and collective .To check up 4 patients with MR techniques of 'black blood' and 'white blood' that were diagnosed by electrocardiogram (ECC) and echocardiogram (ECHO) from 6 living persons in the family. And to measure the area and thickness of VA, then calculate the rate of shortening and thickening of VA's wall and the volume percentage ratio of VA to LV. To analyse the motional state and signal intensity of VA's wall and the cardiac thrombosis.Result: VA involved 13 segments of LV in the familial cases. 2 cases were in apex ,1 case was in apex,anterior and lateral wall , and 1 case was in apex, anterior, lateral,inferior and septum interventriculare. The maximum thickness of VA's wall in systolic phase was 4mm , the minimum was 1mm ,the average was 2.3mm ,while the thickness in diastolic phase was 3.5mm,1mm,2.2mm , respectively. The maximum thickening rate was 12.5%, and the minimum was 0. About the signal intensity of the cardiac muscles of VA's wall, 1 case was normal, 3 cases were low. The maximum volume ratio of VA to LV was 84.5%, the minimum was 8.8%. The motional function of the VA's wall of 2 cases dropped, 1 case was akinetic, and 1 case was dyskinetic. 1 case had only cardiac thrombosis among 4 cases. Conclusion: Familial and hereditary VA of LV has a certain particularity. Because the manifestations of MRI can show pathologic changes of VA, MR is superior to other medical imaging examinations for VA.
