摘要
目的 :为提高先天性尿道下裂一期手术成功率 ,研究设计了带蒂双面包皮皮瓣尿道成形术 (PPDIF) ,并总结分析几种一期尿道成形术的治疗效果。方法 :1988年～ 2 0 0 1年间 ,应用不同方法的一期尿道成形术治疗尿道下裂 190例 ,对比分析单面、双面皮瓣尿道成形术的治疗效果。结果 :阴囊隔皮瓣法尿道成形术的治愈率 92 .9% ( 13 /14 )、游离膀胱粘膜法尿道成形术治愈率 43 .7% ( 7/16)、Duckett术治愈率 83 .3 % ( 3 0 /3 6)、加盖Duckett术治愈率 77.8% ( 2 8/3 6)、Duckett +Duplay术治愈率 5 0 .0 % ( 4 /8)、PPDIF术治愈率 88.9% ( 16/18)、加盖PPDIF术治愈率 94.4% ( 3 4/3 6)、PPDIF +Duplay术治愈率 84.6% ( 2 2 /2 6) ,其中双面皮瓣手术成功率明显高于单面皮瓣。结论 :带蒂双面包皮皮瓣尿道成形术的设计应用为临床提供了一种更好的治疗方法 ,与Duckett术相比 ,本术式能明显降低尿瘘发生率 。
Objective: To increase the successful rate of one-staged urethroplasty, a new operation was designed named pedicled preputial double-faced island flap (PPDIF). The treatment effect of different one-staged urethroplasty was analysed.Methods: From 1988～2001, 190 cases of congenital hypospadias were treated with different one-staged urethroplasty. The effect of using double-faced flap was studied compared with using single-faced flap.Results: 13 cases (92.9%) were cured with island scrotal septum urethroplasty in 14 cases; 7 cases (43.7%) cured with one-staged bladder mucosa urathraoplasty in 16 cases ; 30 cases (83.3%) cured with transverse island flap urethroplasty (Duckett operation ) in 36 cases; 28 cases (77.8%) cured with on-layer Duckett operation in 36 cases; 4 cases(50%) cured with Duckett+Duplay methods in 8 cases. 16 cases (88.9%) were cured with PPDIF; 34 cases (94.4%) cured with on-layer PPDIF operation in 36 cases and 22 cases(84.6%) cured with PPDIF+Duplay methods in 26 cases. Conclusion: The results indicate that PPDIF operation we designed provides a better method for treating hypospadias. Compared with Duckett operation, PPDIF can reduce the urinary fistula occurring rate and make penis better shaped.
