摘要
目的 探讨肿瘤相关物质群 (TSGF)水平对肺癌诊断及转归评估的意义。方法 用比色法检测 16 9例肺癌患者 (治疗前5 8例 ,治疗有效组 5 1例 ,治疗无效组 17例 ,复发转移组 4 3例 )和 134例健康人血清TSGF水平。结果 治疗前、治疗无效及复发转移组患者血清TSGF水平分别为 (76 .4± 18.0 )、(75 .2± 7.0 )和 (74 .6± 12 .0 )U/ml,均明显高于治疗显效组的 (5 7.3±9.3)U/ml及健康对照的 (5 4 .5± 5 .2 )U/ml(P <0 .0 1) ;与局部复发组的 (6 7.6± 9.1)U/ml相比 ,远处转移组患者血清TSGF(78.8± 11.4 )U/ml水平显著升高 (P <0 .0 1)。以 6 4U/ml为临界值时 ,TSGF对肺癌判断的灵敏度、特异度、阳性预测值和阴性预测值 ,分别为 77.6 %、97.0 %、91.8%及 90 .9%。结论 检测血清TSGF水平是肺癌诊断、疗效及预后判断的有效指标。
Objective To determine the significance of serum tumor supplied group of factors(TSGF) level on diagnosis and outcome of lung cancer. Method TSGF was measured by spectrophotometry in 134 healthy controls and 169 patients with lung cancer (58 newly diagnosed tumors, 43 with recurrent or metastatic tumors, 51 responders and 17 non-responders to treatment.) Results The TSGF levels (±s) were (76.4±18.0)U/ml for newly diagnosed cancers, (75.2±7.0)U/ml for non-responders, (74.6±12.0)U/ml for recurrence or metastases, and were profoundly higher than that (57.3±9.3)U/ml for responders and (54.5±5.2)U/ml for healthy controls (P<0.01). Compared with local region relapsed tumors, the TSGF level was obviously increased in distant metastases (78.8±11.4)U/ml vs. (67.1±9.1)U/ml (P<0.01). At the critical value of 64U/ml, TSGF indicated any lung cancer with a specificity of 97%, a sensitivity of 77.6%, a positive predictive value of 91.8% and a negative predictive value of 90.9%. Conclusion The TSGF assay might be a critical parameter for diagnosis, monitoring of response to treatment and predicting prognosis of lung cnacer.
