摘要
目的 :研究金黄色葡萄球菌滤液注射液 (含金黄色葡萄球菌肠毒素C ,SEC)胸腔内注射治疗恶性胸水的疗效。方法 :经组织学或细胞学检查确诊的恶性胸水病人 12例 ,包括非小细胞肺癌 9例 ,小细胞肺癌 ,子宫肉瘤和黑色素瘤各 1例。行常规胸腔穿刺术并尽量抽尽胸水后注入金黄色葡萄球菌滤液注射液 10mL(含SEC 5 0ng) ,每周 2次 ,治疗时间 2~ 3wk。结果 :完全有效 (CR) 10例 (83% ) ,部分有效 (PR ) 2例 ,总有效率 10 0 %。CR中 1例无病生存达 2 0mo ,另有 2例分别在治疗后 10mo和 6mo死亡 ,但未见胸水复发。与治疗前比较 ,胸腔注射金黄色葡萄球菌滤液注射液 6h后普遍出现周围血液淋巴细胞轻度升高 ,而胸水淋巴细胞比例升高显著。未发现明显不良反应。结论 :胸腔内注射金黄色葡萄球菌滤液注射液治疗恶性胸水的疗效确切 。
AIM:To investigate the effect of Staphylococcus aureus culture filtrate injection (containing Staphylococcal aureus enterotoxin A superantigen, SEC) by intrapleural infusion in treatment of malignant pleural effusions. METHODS: Twelve patients with histopathologically confirmed malignant pleural effusions were treated. Nine had non small cell lung cancer, and the rest patients had small cell lung cancer, uterine sarcoma and malignant melanoma respectively. Pleural effusions were removed by conventional thoracentesis and Staphylococcus aureus culture filtrate injection, 10 mL (containing SEC 50 ng) were instilled into pleural cavity twice weekly for 2 3 wk. RESULTS: A total effective rate was obtained in twelve patients, including 10 complete effective (83 %) and 2 partial effective. One of the patients with complete effective had survived 20 mo in a disease free status; two patients died after 10 and 6 mo respectively without signs of fluid reaccumulation. Lymphocyte counts in peripheral blood increased slightly, at the same time there was a significant lymphocytosis in pleural effusions ( P <0.05) six hours after SEC intrapleural instillations compared with to those before treatment. No significant adverse reactions were observed. CONCLUSION:Intrapleural instillations of Staphylococcus aureus culture filtrate injection are highly effective, safe and convenient for use in the treatment of patients with malignant pleural effusions.
