摘要
目的 探讨屈肘坐位牵引法及其治疗肩关节前脱位的临床疗效 ,并与传统的Hippocrates法进行比较。 方法 1 989年 7月—2 0 0 2年 6月急诊整复肩关节前脱位 1 89例 1 94肩 ,随机用本法 1 2 0例 (1 2 4肩 )和Hippocrates法 69例 (70肩 )整复 ,比较整复时间、一次成功率和患者主观痛苦程度 (分为“可以忍受”和“难以忍受”二个等级 ) ,对其中 1 0 6例 (观察组 66例 68肩和对照组 40例 40肩 )首次脱位者进行了 6~ 1 2个月 (平均 1 1 .4个月 )的随访 ,并进行统计学处理。结果 观察组和对照组整复时间分别为 (39.1± 1 7.6)s和(1 1 6 .4± 45 .3)s(P <0 .0 1 ) ;观察组 1 2 0例中 62例表示可以忍受 ,而对照组 65例中仅 1 4例表示可以忍受 (P <0 .0 1 ) ;首次脱位者经过平均 1 1 .4个月的随访 ,观察组肩关节功能Neer评分为 (93 .5± 2 .4)分 ,对照组为 (91 .6± 3 .8)分 (P <0 .0 1 )。结论 与Hippocrates法相比 ,本法具有操作简便、患者痛苦小、关节功能恢复满意等较多优点。
Objective To investigate a method of crooking elbow traction in sitting posture and its effects on reduction of shoulder joint dislocation. Methods 189 patients (194 shoulders) with shoulder dislocation were reduced at emergency department from July 1989 to June 2002. 120 patients (124 shoulders) were reduced by crooking elbow traction in sitting posture and 69 patients (70 shoulders)by Hippocrates. The reduction time, one time successful rate and subjective pain degree of patient (two levels referring as 'bearable' and 'unbearable') were compared in the two groups. Among them, 106 first time shoulder dislocation patients (66 patients (68 shoulders) were in experimental group and 40 patients (40 shoulders) were in control) had been followed up for 6-12 months (average 11.4 months). Results The reduction times for experimental group and control were (39.1±17.6)s and (116.4±45.3)s respectively (P<0.01). 62 of 120 patients in experimental group reported bearable while 14 of 65 reported bearable in control. The first time dislocation patients having been followed-up for average 11.4 months, the shoulder joint function recovery for experimental group were graded at (93.5±2.4) according to Neer score and (91.6±3.8) for control group (P<0.01). Conclusion Compared with Hippocrates, the method of traction in crooking elbow in sitting posture has many advantages such as simply operation, high one time successful rate, less pain of the patient and satisfied joint function recovery.
