颅颈部蛛网膜下腔改变与Chiari畸形发病机理的临床研究 被引量:9

摘要 目的 研究Chiari畸形Ⅰ型(CMI)患者颅颈部蛛网膜下腔(SS)形态学改变及其在疾病发生中的意义。方法 测定病例组和正常对照组颅颈部蛛网膜下腔形态学指标,进行对比分析;在术中观察病例组颅颈部不同组织变异情况。结果 此类患者由于先天性和后天性因素,使整个颅颈部蛛网膜下腔进行性狭窄,其中以枕大池的缩窄最为明显,颈延髓前池和侧池亦有不同程度的缩小,与对照组差异显著。结论 颅颈部蛛网膜下腔进行性狭窄可能在疾病发生发展中起重要作用。 Objective To investigate the alteration of subarachnoid space (SS) in the craniocervical junction (CCJ) region of the patients with type I Chiari malformation (CMI), and to study its clinical significance in the pathogenesis. Methods We reviewed the clinical manifestations and the magnetic resonance image (MRI) information of 34 CMI patients, with 20 normal people as control group. Results Findings in the CMI patients consisted of the narrows of subarachnoid space at the CCJ region, which were aggravated gradually by innate and postnatal factors. Conclusion The narrow of subarachnoid space can cause cerebrospinal fluid circulation disorder at the CCJ region, which plays an important role in the occurrence and development of the disease.
