卵巢过度刺激综合征病人血管内皮生长因子及其mRNA表达的检测

摘要 ①目的 探讨血管内皮生长因子 (VEGF)在卵巢过度刺激综合征 (OHSS)发病过程中的作用及其机制。②方法 用酶联免疫吸附试验 (ELISA)方法 ,测定 14例OHSS病人绒毛膜促性腺激素 (HCG)注射日、胚胎移植日血清及取卵日卵泡液VEGF水平 ;用半定量逆转录多聚酶链式反应 (RT PCR)技术 ,检测取卵日卵巢颗粒细胞VEGFmRNA的表达 ,并与 2 2例非OHSS病人检测结果进行比较。③结果 HCG注射日OHSS组血清VEGF水平与非OHSS组比较 ,差异无显著性 (t=0 .97,P >0 .0 5 ) ;OHSS组胚胎移植日血清、取卵日卵泡液VEGF水平均高于非OHSS组 ,差异有显著性 (t=3.37、2 .19,P <0 .0 5、0 .0 1) ;HCG注射日与胚胎移植日VEGF水平比较OHSS组与非OHSS组差异均有显著性 (t=5 0 .8、2 0 .0 ,P <0 .0 1)。半定量RT PCR结果显示 ,OHSS组VEGFmRNA表达明显增强 (t=3.38,P <0 .0 1)。④结论 VEGF在OHSS发病中起一定作用 。 Objective To explore the role of vascular endothelial growth factor(VEGF) in the pathogenesis of ovarian hyperstimulation syndrome (OHSS) and its mechanism. Methods VEGF levels in 14 OHSS patients' serum collected on the day of injection of human chorionic gonadotrophin (HCG) and embryo-transfer day and in follicular fluid were measured using enzyme-labeled immunsorbent assay (ELISA), the VEGF mRNA expression in granulosa cells detected by RT-PCR and the results were compared with 22 non-OHSS patients. Results The difference of VEGF levels in serum between OHSS group and non-OHSS group on HCG injection day was not significant (t=0.97,P>0.05), but there was significant difference between two groups on embryo transfer day (t=3.37, 2.19; P<0.05, 0.01). There was significant difference between two groups in follicular fluid(t=3.37,2.19; P<0.01,0.05). The comparison of serum VEGF levels between HCG injection day and embryo transfer day all showed significant difference in OHSS group and non-OHSS (t=50.8, 20.0; P<0.01). The results of RT-PCR revealed that the expression of VEGF mRNA in OHSS group was obviously stronger than that in non-OHSS group (t=3.38, P<0.01). Conclusion VEGF has some role in the pathogenesis of OHSS. Measuring VEGF levels in follicular fluid and serum on embryo transfer day can predict the occurrence of OHSS.
