摘要
目的 分析临床上胸腰段及腰骶部脊柱肿瘤误诊的原因 ,探讨解决的方法。方法 回顾性研究我院1997年 1月 - 2 0 0 2年 11月收治的 96例胸腰段及腰骶部脊柱肿瘤患者 ,通过分析其入院诊断 (外院诊断及门诊诊断 )、影像学检查、术后病理诊断 ,探讨误诊原因。结果 96例患者中 ,误诊 32例 ,误诊率为 33.3%。结论 在临床中如遇到胸腰背部或双下肢疼痛的患者首先要排除骨疾患 ,警惕肿瘤的可能性。临床医师应重视物理查体及影像学检查 ,提高相关疾病的鉴别能力 ,及时发现病变 ,提高肿瘤的诊断率 。
Ojectives To analyze the causes of misdiagnosis of the thoracolumbar or lumbosacral spinal tumors and to explore a better way to avoid it.Methods From January 1997 to November 2002, 96 cases of the thoracolumbar / lumbosacral tumors were admitted in our hospital. The data including the primary diagnosis (either of our department’s or of other hospitals’),radiographic findings , postoperative pathologic diagnosis and diagnosis of discharge were all collected to explore the causes of misdiagnosis.Results There were 32 cases who were misdiagnosed in 96 cases . The rate of misdiagnosis was 33.3%.Conclusions The tumor should be considered firstly if other bone disease were excluded to those patients especially with back pain or low extremity pain .Thorough physical examination and radiographic procedures are also important to a clinician. The knowledge of their existence may help clinicians to avoid the misdiagnosis of tumors and improve the accuracy of the clinical diagnosis so as to prevent unnecessary operations.
