摘要
目的 :探索P53在大肠肿瘤中的表达及其与肿瘤的分化、分期和生物学行为的关联性 ,以便揭示其临床病理意义。方法 :应用免疫组织化学方法链霉亲和素—生物素—过氧化物酶 (SABC)法 ,检测新鲜大肠癌及癌旁组织手术标本 78例、大肠腺瘤 87例和正常大肠黏膜 2 6例P53的表达情况 ,应用统计学方法进行分析对比。结果 :(1)癌、腺瘤、癌旁组织和正常黏膜的P53阳性表达率分别是 6 6 7%、6 2 1%、2 9 5 %和 0 ,经行×列 χ2 检验 ,χ2 =5 2 5 0 2 ,P =0 0 0 0 <0 0 1,差异有非常显著性。经分割后检验 :①癌、腺瘤和癌旁组织的P53阳性表达率均高于正常黏膜 (χ2 分别是 34 6 6 7、30 90 8和 9 84 4,P分别是 0 0 0 0、0 0 0 0和 0 0 0 2 ,均<0 0 0 8)。并得出OR分别是 5 3 0 0、4 3 6 76和 11 5 71,均 >1,其 95 %可信区间分别是 6 830~ 4 11 2 96、5 6 72~ 336 30 7和 1 4 86~ 90 10 3,其内均不包含 1。EF分别是 0 6 5 4、0 6 0 7和 0 2 70。②任意两组间P53阳性表达率比较 :癌与腺瘤比较差异无显著性 (χ2 =0 378,P =0 5 38>0 0 0 8) ;癌比癌旁组织和腺瘤比癌旁组织的阳性表达率均高 (χ2 分别是 2 1 5 96和 17 5 42 ,P分别是 0 0 0 0和 0 0 0 0 ,均 <0 0 0 8)。 (2 )腺瘤伴轻、中。
Objective:To explore the expression of P 53 in colorectal tumor and determine the correlations with differentiation,Duke stage and biological action of tumor,to find out its clinicopathological significance.Methods:The expression of P 53 was examined by immunohistochemistry technique(SABC) in 78 specimens of fresh colorectal cancer,paracancerous tissues and 87 adenoma tissues compared with a control group of 26 normal mucoma,then statistical analysis was performed.Results:(1)The frequencies of P 53 expression in colorectal cancer,colorectal adenomas,paracancerous tissues and nomal colorectal mucosa were 66.7%,62.1%,29.5% and 0 respectively and significant difference was found out in expression of P 53 protein among the groups (χ 2=52 502,P=0 000<0 01) with χ 2 test of line×file.① After cut apart,frequencies of P 53 expression in colorectal cancer,colorectal adenoma and paracancerous tissues were equally higher than that of nomal colorectal mucosa (χ 2 were 34.667,30.908 and 9.844, P were 0.000<0.008,0.000<0.008 and 0.002<0.008 respectively),and we can analyze that OR were 53.00>1,43.676>1 and 11.571>1 respectively,Whose confidence interal of 95% were 6.830~ 411.296,5.672~336.307 and 1.486~90.103 respectively,1 was not included in it.EF were 0.654,0.607 and 0.270 respectively;②After cut apart,the frequencies of P 53 expression betwen colorectal cancer and colorectal adenoma were not significantly differenct (χ 2=0.378,P=0 538>0 008) ;After cut apart,frequencies of P 53 expression in colorectal cancer and colorectal adnoma were equally higher than that of paracancerous tissues(χ 2 were 21.596 and 17.542, P were 0.000<0.008 and 0.000<0.008 respectively).(2)The frequencies of P 53 expression in colorectal adenoma with mild,medium and heavy dysplasia were 25%,66.7% and 83% respectively and significant differences were found(χ 2=21 164,P=0 000<0 01) with χ 2 test of line×fine.After cut apart,the frequency of P 53 expression in mild dysplasia was lower than that of medium and heavy dysplasia(χ 2 were 8.854 and 20.417, P were 0.003<0.017 and 0.000<0 017 respectively).There was no significant difference between the frequencies of P 53 expression in medium and heavy dysplasia(χ 2=2 363,P=0 124>0 017) .(3)The frequencies of P 53 expression in colorectal cancer with high,medium differentiation and low+no differentiation were 44.5%,71% and 90% respectively and significant difference was found among the groups(χ 2=11 158,P=0 004<0 01) with χ 2 test of line×file;the frequencies of P 53 expression in high differentiation was lower than that in low+no differentiation (χ 2=10 328,P=0 001<0 017) ;while between high and medium differentiation or medium and low+no differentiation there was no significant difference(χ 2 were 4.185 and, P were 0.041>0.017 and 0.166>0.017 respectively);The frequencies of P 53 expression in colorectal cancer with A,B and C+D by Duke stage were 48.6%,78.3% and 85% respectively and significantly differenct was found among the groups(χ 2=9 573,P=0 008<0 01) with χ 2 test of line×file.After cut apart,the frequencies of P 53 expression in A by Duke stage was lower than that in C+D by Duke stage(χ 2=7 156,P=0 007<0 017) ;while the frequencies of P 53 expression in A to B by Duke stage and B to C+D by Duke stage were not significantly differenct(χ 2 were 5.112 and, P were 0 024>0 017 and 0 704>0 017 respectively).(4)The frequencies of P 53 expression with and without invasion of serous membrance or lymph node metastasis were not significantly differenct(χ 2 were 0.411 and 0 293, P were 0 52>0 05 and 0 588>0 05 respectively).Conclussions:The significant expression of P 53 was found in colarectal cancer,colarectal adenoma and paracancerous tissues,The expression P 53 in colorectal carcinogenesis and adenomas canceration is more earlier than morphological change;The expression of P 53 is correlated with differentiation and Duke stage,whi
