摘要
目的 探讨妇女在体外受精 -胚胎移植 (invitrofertilizationandembryotransfer ,IVF -ET)周期中血清及卵泡液中白介素 - 1α、β(IL 1α、IL 1β)水平变化规律及其与着床率的关系。 方法 选取 2 0 0 0年 7月至 9月南方医院生殖中心行IVF -ET患者 36例。收集肌注HCG日、取卵日、胚胎移植日的血清和卵泡液 ,采用定量酶联免疫方法检测血清、卵泡液中IL 1α、IL 1β水平 ,利用放射免疫法测定IVF -ET过程中不同时段血清雌二醇 (E2 )、卵泡刺激素 (FSH)、黄体生成激素 (LH)、孕酮 (P)水平 ,按是否胚胎着床分组 ,对两组临床资料、激素和IL 1α、IL 1β水平进行回顾性相关分析。结果 IVF -ET中 ,着床组的卵泡液IL 1β水平明显高于非着床组(P <0 0 0 1) ,在HCG日或移植日 ,两组血清IL 1β浓度相比差异有显著性意义 (P <0 0 5 )。卵泡液及HCG日血清IL 1β水平与卵细胞成熟度相关 (r =0 74 ,P <0 0 1;r =0 81,P <0 0 1) ,HCG日血清IL 1β水平与血清E2 正相关 (r =0 83,P <0 0 1)。血清及卵泡液IL 1α水平两组差异均无显著性意义 (P >0 0 5 )。结论 IL 1β与E2 、卵细胞成熟度相关 ,血清IL 1β参与卵泡的生长发育 ,调控卵细胞成熟及内膜生长 ,参与IVF -ET胚胎着床过程 。
Objective To study the levels of interleukin 1α(IL 1α)and interleukin 1β(IL 1β)in human serum and follicular fluid, and to evaluate the relationship between IL 1 level and the implantation rate of in vitro fertilization-embryo transfer (IVF-ET).Methods The serum and follicular fluid (FF) of the thirty-six patients from July to September 2000 in the Reproduction Center of Nanfang Hospital undergoing IVF ET were collected on the day of HCG intramuscular injection,ovum taking,and embryo transferring,respectively.IL 1 level in serum and follicular fluid was measured by quantitative ELISA.Follicle stimulating hormone (FSH),luteinizing hormone (LH),estradiol (E 2 ) and progesterone (P) in serum were measured with immunoradiometric assay.According to result of their implantation,IVF-ET patients were divided into the implantation group or non-implantation group. Results The difference between the level of IL 1α in serum and FF in the two groups was not statistically significant (P>0 05).The concentration of FF IL 1β in the implantation group was significantly higher than that of the other group (P<0 001).The concentration of FF IL 1β was positively correlated with maturation degree of oocytes (r=0 74,P<0 01);serum concentrations of IL 1β on the day of HCG administration was positively related to serum E 2 (r=0 83,P〈0 01).Conclusion IL 1β level is related to E 2 level and oocyte maturation——it plays a role in affecting oocyte development and maturation and embryo implantation of IVF ET,thus affecting the implantation rate.
