摘要
目的 讨论颈动脉球体瘤的诊断手段、术前准备和手术方法。方法 总结诊治8例颈动脉球体瘤的经验。以术前磁共振、血管造影明确诊断,采用动脉外膜下剥离及颈外动脉切除,彻底切除颈动脉球体瘤。结果按Shamblin分期3例为Ⅱ期,5例为Ⅲ期,有家族史者3例。5例Ⅲ期的病人,采用了动脉外膜下剥离及颈外动脉切除的方法,均保全了颈总和颈内动脉,瘤体获全切,随访未见肿瘤复发。结论 本文介绍的手术方法可以成功地切除颈动脉球体瘤。
Objective To explore the diagnostic methods pre-operative examination as well as surgical techniques of paraganglioma Methods The clinical data of paraganglioma in eight cases treated in our hospital were reviewed retrospectively. MRI plus angiography were helpful for correct diagnosis. Paraganglioma in all eight cases were resected completely by dissection under arterial adventitia and removal of external carotid artery. Results According to Shamblin' s staging system, three cases belonged to stage Ⅱ, five belonged to stage Ⅲ. Of which, three cases were found to have a family history. All five paragangliomas from stage Ⅲ were completely resected by the methods mentioned above, therefore the internal carotid artery and common acrotid artery were persevered. Follow-up study showed no relapse. Conclusion The surgical technique introduced here was useful for complete removal of paraganglioma.
