一氧化碳中毒在急性肺损伤中对肺通气灌流的影响

摘要 本研究旨在了解迭加CO中毒对吸入性肺损伤时肺气体交换的影响。10只麻醉犬经气管内注入盐酸导致吸入性损伤。5只犬作对照,另5只吸入CO混合气造成CO中毒,监测肺通气,灌流改变。酸吸入引起肺分流及低V/Q区肺血流增加;迭加CO中毒加剧了通气、灌流失衡,肺血流从低V/Q区转向分流区,使低氧血症更趋恶化。 The purpose of this study was to evaluate the influence ofcarbon monoxide poisoning on pulmonary gas exchange in acute lung injury.Hydrochloric acid was injected intratracheally in ten anesthetized dogs to produceinhalation lung injury. Five dogs were ventilated with 1% CO in air for tenminutes to produce CO poisoning, and the others were used as control.Ventilation-perfusion distributions were monitored. Acid aspiration causedsignificant shunting and perfusion to low V/Q units. Superimposed CO poisoningaccentuated V/Q inequality with a conversion of low V/Q units to shunt, leadingto severe hypoxemia.
