腰椎板间撑开减压椎管成形术的解剖学基础 被引量:3

摘要 目的:探讨腰椎椎板间撑开减压椎管成形手术的解剖学基础。方法:采用3具尸体标本和4具腰椎标本,模拟实际手术情况,测量椎板间隙撑开前后手术野的变化及对术中减压操作的影响。结果:椎板间撑开的主要活动部位在腰椎小关节面和腰椎间盘,椎板间撑开幅度平均(0.93±0.24)cm,可满足手术需要,L_(3~4)、L_(4~5)间隙较L_5~S_1更易撑开(P<0.05)。结论:腰椎板间撑开技术是可行的,能有效扩大手术野,满足减压操作需要。 Objective: To testify the anatomical feasibility of distractve laminoplasty for decompression of lumbar stenosis. Methods: Three cadaveric bodies and 4 fresh- frozen lumbar specimens were used. After operation by the procedure described by O'Leary, changes of laminae interspace were measured'and fields for operative management were evaluated. Results: Distraction occurred at the facet joints and the disc. The average increasment of laminae interspace was (0. 93 ± 0.24)cm, with L3-4, L4-5 easily be distracted than L5/S1 . Conclusion: Distraction between Laminae is technically practical for enlargement of operative fields and can meet the need of decompressive procedure.
