摘要
目的 探讨腹膜后腔镜在肾盂整形术的应用价值。 方法 对比分析腹膜后腔镜 (6例 )、开放Anderson Hynes肾盂整形术手术 (10例 )两组手术时间、住院时间、术中出血及住院费用 ,术后症状变化和B超、IVU复查结果。 结果 16例均成功完成手术 ,腹腔镜组无中转手术者。腹腔镜组手术时间为 (2 79± 5 2 )min ,开放手术组为 (12 1± 4 4 )min(P <0 .0 0 1) ;腹腔镜组住院时间、术后住院时间、术中出血均较开放手术组少 ,术后漏尿发生率高于开放手术组 ,但两组比较差异无显著性意义 (P >0 .0 5 )。两组术后均未发生严重并发症。术后随访 3个月 ,腰部疼痛缓解两组比较差别无显著性意义 ,腹腔镜组均恢复正常工作和生活 ,而开放手术组仅 5例恢复正常工作和生活。 结论 腹腔镜下Anderson Hynes肾盂整形术可以达到开放手术的治疗效果 ,又具备了微创手术恢复快的特点。
Objective To evaluate retroperitoneal laparoscopic Anderson-Hynes pyeloplasty. Methods 6 patients underwent laparoscopic and 10 patients underwent open Anderson-Hynes pyeloplasty.The clinic outcome including subjective and objective findings was compared between the two groups. Results All the procedures were successfully completed.The mean operating time was 279 minutes for laparoscopic pyeloplasty being much longer than the open procedure 121 minutes(P<0.001).The mean hospital stay,operating bleeding volume,postoperative complication and hospital cost were about the same in the two groups(P>0.05).No severe postoperative complications were found.3 months after the procedure,the pain-free rate was about the same between the two groups.However the open surgery group had much pain and prolonged convalescence due to the flank incision. Conclusions Laparoscopic pyeloplasty is a minimally invasive technique that provides similar clinical and radiographic results as open pyeloplasty.
