摘要
目的 评价低位直肠癌保肛术保留植物神经后对排尿功能、性功能及局部复发的影响。 方法 根据直肠癌Dukes不同分期 ,对A、B期 38例实施完全保留神经 ,对C期 8例实施保留一侧神经或仅保留骨盆神经 ,并用双吻合器行低位保肛术。 结果 A、B期保留神经者 92 .4 %能勃能 ,10 0 %能射精 ,近期的排尿障碍 2例 ,C期切除神经者 ,4例勃起障碍 ,5例射精障碍。吻合口瘘 2例、局部复发 2例。 结论 保留植物神经的低位直肠癌保肛手术能有效地减少术后排尿功能和性功能障碍 。
Objective To evaluate the effect on postoperative urinary and sexual function and local recurrence of the surgery of anal sphincter and autonomic nerve preserving for low rectal cancer. Methods The surgery of anal sphincter and autonomic nerve preserving was performed on 46 cases of low rectal cancer.Nerve were fully preserved in 38 cases of Dukes A,B stage,one side nerve preserved in 6 cases of stage C,or only pelvic nerve preserved in another 2.The anastomosis was performed with double stapling procedure after proctocolectomy.Results Erection and ejaculation remained in Dukes A,B in 92.4%,100%;2cases suffered temparary urinary dysfunction;4 cases suffered erection dysfunction,sexual function were lost in 5 cases of C stage.There were 2 cases suffered from anastomotic fistula and 2 cases from local recurrence.Conclusion The surgery with anal sphincter and autonomic nerve preserved for low rectal cancer is effective to reduce the postoperative urinary and sexual dysfunctiom,while maintaining a better life qulity.
