低蛋白饮食对糖尿病肾病大鼠肾脏肥大的影响及其机制探讨 被引量:1

Effect of low-protein diet on renal hypertrophy of diabetic glomerulus and its relationship with cyclin kinase inhibitor P27 level

摘要 目的 :探讨低蛋白饮食对糖尿病肾病大鼠肾脏肥大的影响及其与肾小球周期素激酶抑制剂 P2 7的关系。方法 :腹腔注射链脲佐菌素建立糖尿病 (DM)模型 ,分别予正常蛋白 (NPD组 ,n=6 )、低蛋白 (L PD组 ,n=6 )饮食 8周 ,同时对正常大鼠(正常对照组 ,n=6 )予正常蛋白饮食 8周。蛋白印迹方法测定肾小球裂解液 P2 7蛋白水平 ,EL ISA方法测定肾小球裂解液细胞外基质 (ECM)蛋白 ( 型胶原及纤维连接蛋白 )和尿白蛋白 ,氧化酶法测定血糖 ,溴甲酚绿法测定血白蛋白 ,采用图像分析系统测定肾小球面积。 结果 :与正常对照组相比 ,NPD组和 L PD组大鼠肾小球 P2 7水平及 ECM蛋白水平均增高 ,尿白蛋白排泄增多 ,肾质量 /体质量比例增高 ,肾小球面积增大。同 NPD组相比 ,L PD组大鼠肾小球 P2 7水平降低 [(11.1± 3.6 ) vs(19.6± 5 .1) ,P<0 .0 1],纤维连接蛋白水平 [(9.8± 2 .9) vs (17.1± 3.9) pg/m g,P<0 .0 1]及 型胶原水平 [(99.6± 2 3.5 ) vs(15 0 .6± 4 7.1) pg/mg,P<0 .0 1]下降 ,尿白蛋白排泄减少 [(18.13± 3.2 3) vs (4 0 .18± 3.79)μg/2 4 h,P<0 .0 1],大鼠肾质量 /体质量比例下降 [(12 .0 3± 1.85 ) vs (15 .6 2± 2 .13) ,P<0 .0 1],肾小球面积减少 [(72 1± 75 ) vs (815± 97)μm2 ,P<0 .0 1]。 Objective: To investigate the relationship between the cyclin kinase inhibitor P27 and the effect of low protein diet on the renal hypertrophy of diabetic glomerulus. Methods: STZ induced diabetic rats were given low protein diet or normal protein diet for 8 weeks. The P27 protein of glomerular lysate was detected with Western blotting analysis. The extracellular matrix (ECM) protein (type Ⅳ collagen and fibronectin) of glomerular lysate and 24 h urine albumin was examined with ELISA. Image analysis system was used to detect area of each glomerulus. Results: Glomerular P27 protein increased in diabetic rats no matter what kinds of diet they were given. Meanwhile, the 24 h urine albumin excretion, glomerular ECM, glomerular area elevated and so did the ratio of kidney mass over body mass in diabetic rats. Glomerular P27 level [(11.1± 3.6) vs (19.6±5.1), P <0.01],24 h urine albumin excretion [(18.13±3.23) vs (40.18±3.79) μg/24 h, P < 0.01], glomerular area [(721±75) vs (815±97) μm 2, P <0.01] and the ratio of kidney mass over body mass [(12.03±1.85) vs (15.62±2.13), P <0.01] were lower in diabetic rats with low protein diet than diabetic rats with normal protein diet. Glomerular P27 level was linearly related with the ratio of kidney mass over body mass in diabetic rats. The blood glucose and serum albumin remained constant in diabetic rats no matter what kinds of diet they were given. Conclusion: Low protein diet can inhibit glomerular hypertrophy and may protect diabetic glomerulus through decreasing glomerular P27 level.
