不同年龄烧伤患者增生性瘢痕的胶原构成及影响因素的研究 被引量:9

摘要 目的 观察不同年龄患者增生性瘢痕 (HS)胶原构成及相关因素的变化 ,并探讨其规律。 方法 HS患者 30例 ,按年龄分为 1~ 19岁组和 2 0~ 5 0岁组 ,取患者对侧部位的正常皮肤(NS)作对照组。采用原位杂交、链酶亲和素 过氧化物酶复合物免疫组织化学方法 (SABC)及图像分析法 ,观察胶原比例和转化生长因子β1(TGFβ1)、胶原酶 (MMP 1)、金属蛋白酶组织抑制剂 (TIMP 1)的变化。结果  1~ 19岁HS组Ⅰ /Ⅲ型胶原比例平均为 6 .4 8,2 0~ 5 0岁HS组为 3.76 ,但两个年龄组各病程之间Ⅰ /Ⅲ型胶原比例均无明显差异。 1~ 19岁HS组TGFβ1呈高表达 ,与 2 0~ 5 0岁HS组比较 ,差异有非常显著性意义 (P <0 .0 1)。TIMP 1mRNA表达各年龄组之间无明显差别 ,HS组的表达显著高于NS组 ,MMP 1的表达明显低于TIMP 1,与NS组MMP 1的表达比较 ,差异无显著性意义 (P >0 .0 5 )。 结论  (1)HS中TGFβ1的表达与年龄呈负相关 ,TGFβ1的高表达促使Ⅰ /Ⅲ型胶原比例增高甚至比例失调。 (2 )高表达的TIMP 1通过抑制MMP 1的表达促使HS形成 ,其表达与年龄无明显相关性。 Objective To investigate the collagen constitution of hyperplastic scar (HS) in different ages and the change of relative factors. Methods Thirty cases with HS were divided into two groups according to patients' age: group 1 (1 19 years, A) and group 2 (20 50 years, B). The normal skin (NS) from corresponding age of volunteers was employed as control group. The changes in TGFβ1, collagenase (MMP 1) and tissue inhibitor of metalloproteinase (TIMP 1β) and the collagen ratio were observed by means of in situ hybridization technique and SABC (Strept Avidin Biotin complex) immunohistochemistry and image analysis. Results The ratio of type Ⅰ to type Ⅲ collagen in A group was 6.48 in average and 3.76 in B group, but there was no evident difference in the ratio during the disease process in both groups. The expression of TGFβ1 in A group was much higher than that in B group (P<0.01). The TIMP 1 mRNA expression showed no difference among all age groups in HS patients, but it was much higher than that in NS group. The MMP 1 expression was evidently lower than TIMP 1 expression,and there was no difference in MMP 1 expression compared with NS group. Conclusion (1) The TGFβ1 expression in HS patients was negatively correlated with age,and the increased expression of TGFβ1 produced an increase ratio of type Ⅰ to type Ⅲ collagen . (2) High level expression of TIMP 1 led to the formation of HS by inhibiting MMP 1 expression,and the expression was not related to age. [
