摘要
目的 探讨乳癌术后皮瓣坏死的原因及其防治。方法 对我院 1992年 12月至 2 0 0 2年 12月手术治疗的 4 2例乳腺癌病人的临床资料进行回顾性分析。结果 4 2例乳腺癌病人术后发生皮瓣坏死 17例 ,发生率为 4 0 6 %。结论 乳癌术后皮瓣坏死的因素是多方面的 ;术中护好皮瓣血运 ,术后适当胸部加压包扎 ,保持引流管通畅 ,及时抽除皮下积液。一旦发生切口感染 ,及时拆线引流。发现皮瓣坏死 ,及时剪除换药 ,必要时植皮 。
Objective To study the causes,prophylaxis and therapy of skip flap necrosis following operation of breast cancer.Methods The clinical data of 42 patients with breast cancer received surgical treatment from December 1992 to December 2002 was analysed retrospectively.Results Fourty two patients with breatst cancer were received surgical treatment.The skin flap necrosis was found in 17 cases and the necrosis rate was 40.6%.Conclusion The causes of necrosis of skin flap are various.In the course of operation,the blood transport of skin flap should be protected.After operation,the chest should be packed properly,the drainage tube should be kept unobstructed and the liquid under skin flap must be taken out in time.When the incision was infected,the stitches of the incision must be taken down and drained in time.When the skin flap necrosed,the necrosis tissue muse be cut down in time.And if necessary,the healing time could be shortened with skin transplantation.
