摘要
选用供心 ,在全麻体外循环下 ,切除病心 ,进行原位心脏植入。围术期进行血流动力学监测及免疫抑制治疗 ,观察排斥反应。供心冷缺血时间 180 min,受体主动脉阻断 72 min,移植后心脏自动复跳 ,辅助循环 30 min后 ,顺利脱离体外循环 ,血流动力学稳定。心脏移植的成功取决于正确选择患者、采用合适的手术方式、良好的供心保护方法。
After donor heart for transplant was obtained,the diseased heart was removed and implanted donor heart into original place under general anesthesia and extracorporeal circulation. During the process the hemodynamics was monitored; immunosuppressor therapy was carried out and rejective response was observed.The cool ischemic time of donor heart was 180 min; the aortic arressted time of recipient was 72 min. After transplantation the donor hearts started beating again. The transplantation hearts got rid of cardiopulmonary circulation after 30 min assistant extracorporeal circulation. The hemodynamics was steady.The success of heart transplant lies on right choice of patient, using appropriate operative technique, good myocardial protection of donor heart, perioperative management and right application of immunosuppressor.
