摘要
目的 :研究肺心病患者氧化 -抗氧化系统、血小板及肺血管内皮细胞功能变化及相互关系 ,探讨益气活血组方治疗肺心病的疗效和作用机制。方法 :随机选取肺心病患者 5 7例 ,随机分为治疗组 (30例 )及对照组 (2 7例 ) ,对照组给予常规治疗 ,治疗组除常规治疗外加用益气活血制剂。另选取 13例为健康对照组。记录所有患者的临床疗效 ,检测治疗前后血 L PO、SOD、GMP14 0、CEC、Pa O2 和 Pa CO2 的变化 ,并进行各指标间的相关分析。结果 :1治疗组显效率及 Pa O2 较对照组明显增高。 2两组肺心病治疗前 L PO均升高 ,SOD明显降低 ;治疗前 GMP14 0、CEC均升高 (P均 <0 .0 1) ,治疗后均下降 (P均 <0 .0 1)。益气活血治疗组上述变化较对照组明显 ,两组比较有显著性差异 (P均 <0 .0 1)。 3相关分析结果 ,CEC数量与 Pa O2 呈负相关 ,与 L PO呈正相关 ,与 SOD呈负相关 ,与血小板膜 GMP14 0呈正相关。血小板膜 GMP14 0与 L PO呈正相关 ,与 SOD呈负相关 ,与 Pa O2 呈负相关。结论 :益气活血治疗通过恢复氧化 -抗氧化系统的平衡 ,从而保护肺血管内皮细胞和血小板 。
Objective:To observe the clinical effects of Yiqi Huoxue prescription on chronic pulmonary heart disease and explore its partial mechanism by determin the level changes of oxidative and antioxidant system,endothelial cells and platelets.Methods:Fifty seven patients were divided into treatment group and control group.The control group received regular management,and the treatment group with combination of Yiqi Huoxue prescription.Some indexes were measured respectively before and after treatment.The results were compared with those of healthy persons.All patients were examined the arterial blood gas and analyzed the relations among the oxidative and antioxidant system,endothelial cells and platelets functions.Results:①the effective rate of the treatment group is 86.6%,higher than that of the control group(72.2%,P<0.05).②The blood levels of LPO,GMP140,CEC in all patients before therapy were higher than those of the health(all P<0.05),and there was a decrease of those after treatment(all P<0.01).The level of SOD in all the patients before therapy was lower than that in the health and increased after treatment.③The increase of SOD in the treatment group was more than that of the control group obviously.The decrease of LPO,GMP140,CEC in the treatment group was more than those in the control group(all P<0.01).④PaO 2 were improved in both the patient groups,and there was more significant improvement in the treatment group(P<0.01).⑤CEC as well as GMP140 was adversely correlated to PaO 2(r= 0.9234,P<0.01) and SOD(r= 0.8634,P<0.01).There was positively correlation between CEC and GMP140(r= 0.7591, P<0.01).Conclusion:Yiqi Huoxue prescription in treatment of chronic pulmonary heart disease displays extraordinary effects by balanced the oxidation system and anti oxidation and curing the damage of pulonary endothelial cells and activating platelets.
