摘要
目的分析囊性脑膜瘤形成的机理、诊断及手术要点。方法回顾性分析3例囊性脑膜瘤的临床和影像学资料,结合文献讨论术前诊断、病因以及外科治疗。结果MRI和CT常发现近硬膜的囊性病变伴或不伴囊壁强化,术中切除瘤结节及其附着的硬膜,在不损伤脑功能的情况下尽可能切除囊壁。结论准确的术前诊断和术中病理诊断是制定外科手术策略和获得良好预后的基础。
Objective To explore the pathogenetic mechanism of cystic meningiomas and the keys to its diagnosis and operation.Methods The clinical and imaging data of3patients with cystic meningiomas were analyzed retrospectively and the preoperative diagnosis,etiology and surgical treatment of the cystic meningioma were discussed.Results Some of the cystic lesions close to the dura had enhancement of a cystic walls and the others did not on MRI and CT.The tumors and dura adhering to the tumors were totally removed.The cystic wall was resected as totally as possible without injury to normal brain tissue.Conclusion Accurate preoperative radiological diagnosis and intraoperative pathological examination are fundamental to the surgical strategy and prognosis in the patients with cystic meningiomas
