摘要
目的 :探讨非霍奇金淋巴瘤 (NHL)中Bcl 6的表达及生物学意义。方法 :应用免疫组化S P法检测 13 3例NHL病理标本中Bcl 6的表达 ,对 5 3例死于NHL的患者进行生存率分析。结果 :Bcl 6在B细胞淋巴瘤 (BCL)和T细胞淋巴瘤(TCL)中阳性率分别为 5 4.8%( 5 1/93 )和 2 .5 %( 1/4 0 ) (P <0 .0 1) ;BCL中主要表达于滤泡性淋巴瘤 (弱阳性 ,阳性率 10 0 .0 %)和弥漫性大B细胞性淋巴瘤 (强阳性 ,阳性率 5 2 .0 %) ,两者阳性率与表达强度比较差异均有非常显著性 (P <0 .0 1)。TCL中仅 1例CD 3阳性外周T细胞性淋巴瘤表达Bcl 6。 5 3例随访病例 ,Bcl 6阳性组平均生存时间 18.9月 ,阴性组平均生存时间 3 2 .1月 ,两组比较差异有显著性 (P <0 .0 5 )。结论 :Bcl 6表达于滤泡中心细胞来源的BCL和一些特殊类型的TCL 。
Objective:To investigate Bcl 6 expression and its biological significance in non Hodgkin’s lymphoma(NHL).Methods:Bcl 6 was detected by SP immunohistochemical technique in 133 patients with NHL,and the survival rate was analyzed in 53 subjects died of NHL.Results:Bcl 6 expression rates in B cell lymphoma(BCL) and T cell lymphoma(TCL) were 54.8%(51/93) and 2.5%(1/40) respectively,and there was a significant difference( P< 0.01).In BCL,expression rate of Bcl 6 was higher in follicular lymphoma than that in diffuse large BCL (100% vs 52%, P< 0.01),but its expression intensity was just the reverse(weak vs strong, P< 0.01).Only one case of CD3 + peripheral TCL appeared positive staining of Bcl 6.In 53 dead cases,the survival time in patients with negative staining of Bcl 6 was superior to that with positive expression(32.1 vs 18.9 months, P< 0.05).Conclusion:Bcl 6 protein is expressed in BCL originated from follicle central cells and few subtypes of TCL,and its over expression might be a cause of malignant transformation and an indicator for bad prognosis in some subtypes of lymphoma.
