翼状胬肉手术前后的角膜地形图改变 被引量:4

摘要 目的:利用角膜地形图,观察翼状胬肉对角膜表面屈光度的影响。方法:原发性翼状胬肉44例52只眼。裂隙灯显微镜下测量其水平向伸入角膜的距离(横径)及跨过角巩膜缘时的弦长(竖径);手术前后角膜地形图检查记录Simk值(最陡径与最平径值、散光值及轴向)、角膜中心屈光度和平均屈光度以及以胬肉尖部为中心的扁平区内的最小屈光度(p-屈光度);比较胬肉大小对角膜散光的影响以及各屈光度的差别,并观察胬肉术后角膜的屈光变化。结果:术前,角膜地形图显示一扁平区,位于鼻侧的水平径向上,角膜呈循规性散光,散光度与胬肉的竖径及横径呈正相关(r=0.42,P=0.038;r=0.58,P=0.002),与胬肉距中心的距离呈负相关(r=-0.68,P=0.0002);p-屈光度犤(38.67±2.44)D犦显著低于(P<0.0001)角膜平均屈光度犤(43.76±1.50)D犦、中心屈光度犤(43.98±1.48)D犦及最平径值犤(42.69±1.52)D犦。术后,原胬肉所在处角膜屈光度增高(P<0.01),角膜平均屈光度增加,散光度减小(P<0.05)。结论:胬肉到达瞳孔光学区前已影响视功能,且治疗时间可影响视功能受损程度及术后恢复程度;手术有利于角膜屈光状况的恢复。 Objective:To study corneal refractive changes before and after pterygia operation by means of corneal topography.Methods :All of52eyes with primary pterygia were measured for the horizontal and vertical extensions by slit-lamp microscope.Mean corneal power,central corneal power,the flattest meridional power as well as the astigmatism were recorded for each keratography.Pterygium-associated flattening was also recorded as the refractive power and its location in regard to the center.Paired t-test and Pearson correlation in stasitica were used for analysis.Results:The keratography ex-isted a with-the-rule astigmatisM [(1.92±1.46)],its value had a negative correlation with pterygium-dis-tance,and had a positive correlation with the verticle and horiztontal extensions of the pterygium;ptery-gium-induced power[(38.67±2.44)D]was significantly lower(P<0.0001)than the mean corneal power[(43.76±1.50)],central corneal power[(43.98±1.48)d]and power of the flattest meridian[(42.67±1.32)].After operaton,corneal refraction in pterygia site and mean corneal refraction increased,corneal astigmatism decreased.Conclusion:Pterygium can induce an area of corneal flattening central to its apex.The cornea had a with-the-rule astigmatism whose value increased as pterygium approached the corneal center.Exertion of pterygia can help cornea return to normal refractive state.
