摘要
目的 探讨CT双期增强检查对肾淋巴瘤的诊断价值。方法 分析 8例肾淋巴瘤的CT表现及特征 ,其中6例经CT检查并手术证实 ,2例经穿刺病理证实 ,所有病例均先行CT平扫 ,平均层厚为 10mm ,PITCH为 1,后做双期动态增强扫描。结果 病灶形态 ,大小及界限 :4例呈局灶改变 ,其中单发结节 3例 ,多发结节型 1例 ,1例内可见多个散在钙盐状钙化影 ,表现弥漫浸润型 4例 ;8例病灶界限均为不清。周围累及情况 :肾门区淋巴结肿大 6例 ,肾上腺受累 5例 ,合并肾上腺肿块 1例 ,同侧腰大肌受侵 3例。病灶密度及强化方式 :病灶平扫显示弥漫浸润型为均匀稍高密度 4例 ,平均CT值为 (4 5 .8± 10 .1)HU ;结节型均为稍低密度 ,动态增强皮髓交界期病变轻度强化 ,平均CT值为 (5 5 .3± 9.1)HU ;实质期为轻中度强化 ,强化均匀 ,强化程度较皮髓交界期明显 ,平均CT值为(6 6 .7± 12 .1)HU。结论 双期动态增强有利于与其他恶性肿瘤鉴别 ,双期更易于显示肾动、静脉包绕、推移及变细征象 ,故双期动态增强CT对肾淋巴瘤的诊断、肾皮髓受侵程度 (分期 )
Objective To assess the value of dual phase contrast enhanced CT examinations in diagnosis of renal lymphoma.Methods CT scans were performed on 8 patients with renal lymphoma,all cases were confirmed by surgery or biopsy in our hospital.CT technique:CT examinations were performed with scanner(siemens plus4),CT scans were generally performed with 7 mm thick sections,pitch of 1.0,Then all cases underwent double phase dynamic contrast enhanced CT scans after intravenous administration of 90～100 ml bolus of iothalamate meglumins with a power injector through an arm vein at 3ml sec.Results 14 cases of renal lymphoma showed local lesions ,solitary nodules in 3cases and multiple nodules in one case;multiple flecked cacification,can be seen in 1case;4 cases has diffuse infiltration.the of 8 cases was unclear 2,6 cases have mphadenopathy of hilum of kidney,renal gland was involved in 5 cases ;1 cases has adrenal masses ;the involvement of ipsilateral psoas musule can be seen in 3 cases.The density and ways of contrast enhancement of the lesions,4 cases showed diffuse infiltrates type of density in CT scans,their appearances of contrast enhanced CT scans are high density and very,average CT value is 45.8 +18HU,type of nodes is a litter low density.Conclusions The cortical phase of dual phase dynamic contrast enhanced CT scans was helpful in evluating the degree of Renal lymphoma,parenchymal phase was useful in differenting other maligalnt tumors.Therefore,dual phase dynamic contrast enhanced CT scans had important value in diagnaosing and differentiating renal lymphoma and the degree of renal cortical and medullary matter invasion.
