摘要
目的 :探讨卵泡发育及宫颈黏液中肿瘤坏死因子α的表达与黄体功能不全患者不孕、不育的关系。方法 :对 2 1例经基础体温及黄体期血清孕酮测定诊断为黄体功能不全的患者 ,B超监测卵泡发育、用放射免疫法 (RIA)检测排卵期血清、宫颈黏液中肿瘤坏死因子α(TNF-α)的表达 ,用化学发光免疫分析仪测定黄体期血清孕酮水平 ,并与 1 8例正常组作对照。结果 :黄体功能不全患者 (L PD组 )与对照组年龄比较无统计学意义 (P >0 .0 5 ) ,两组优势卵泡直径比较差异有统计学意义 (P <0 .0 5 )。两组黄体中期血清孕酮比较差异有统计学意义 (P <0 .0 1 )。优势卵泡直径与黄体中期血清孕酮呈正相关 (P <0 .0 1 )。排卵期L PD组血清 TNF-α与对照组比较差异无统计学意义 (P >0 .0 5 ) ,而宫颈黏液中 TNF-α与对照组比较差异有统计学意义 (P<0 .0 1 ) ,且宫颈黏液 TNF-α与黄体中期血清孕酮呈负相关 (P <0 .0 1 )。结论 :卵泡发育异常导致黄体功能不全。排卵期生殖道局部肿瘤坏死因子α的高表达可能是黄体功能不全患者不孕、不育的发病环节中的主要免疫因素之一 。
Objective:The purpose of this study is to probe into the relationship between infertility due to luteal phase defect and ovarian follicular development and the levels of tumor necrosis factor α(TNF α) in cervical mucus Methods:We studied 21 infertile patients with luteal phase defect (LPD group) and 18 normal fertile women (control group) Ovarian follicular development was detected by ultrasonography. Serum and cervical mucus levels of TNF α levels were determined by radioimmunoassay(RIA). Serum progesterone(P) levels from the midluteal phase were measured by Access Immunoassay System.Result:The follicular diameter was significantly smaller in the LPD group than those in the control group(P< 0.05).Serum P level from the midluteal phase in LPD group was significantly lower than those in the control group(P<0.01).While there were significant positive correlations between maximum follicular diameter and midluteal serum P(r=0.691,P<0.01). During ovulatory phase,the levels of TNF α were similar in serum of the control group and the LPD group(P>0.05). TNF α levels in cervical mucus in the LPD group were significantly higher than those in the control group(P<0.01). Of great interest was the negativecorrelationbetweenTNF αlevelsincervicalmucusandmidlutealserumP(r=-0.851, P< 0.01).Conclusion:These findings support the concept that aberrant folliculogenesis are associated with LPD. The increase of TNF α levels in genital locality may play an important role in infertility with LPD during ovulatory phase, and thus may affect indirectly corpus luteum resulting in LPD.
