摘要
目的 报告风心病换瓣术后置入永久起搏器的护理措施和体会。方法 回顾性分析 3 0例风心病换瓣术后置入永久起搏器的患者的随访资料。其中男 11例 ,女 19例 ;平均年龄 ( 5 1± 12 )岁。因持续性房颤伴和R R间期 >3s或III度房室传导阻滞 (AVB)( 2 2例 )、严重窦性心动过缓或III度AVB( 8例 )分别在换瓣术后 2个月～ 15年置入永久起搏器。结果 3 0例患者永久起搏器安置均成功 ,术后平均随访 ( 2 6.14± 2 0 .2 5 )月 ,无穿刺部位和囊袋血肿或感染、电极脱位或功能异常等并发症 ,起搏器的感知和起搏功能均良好。结论 换瓣术后安置永久起搏器的护理重点是术前加强心理护理、围术期正确调控抗凝、术后加强心电监护和健康宣教 。
Objective To report the nursing experience with permanent pacemaker (PP) implantation after cardiac valvular replacement.Methods The follow up data of 30 patients (11 male and 19 female, mean age 51 years) ,who received a PP from 2 months to 15 years after cardiac valvular replacement, were analyzed retrospectively. A PP was inserted through right vena cephalica in 22 cases and vena subclavia in 8 cases, which was due to permanent atrial fibrillation with R R interval >3 s or complete atrioventricular block (AVB) in 22 cases and severe brachycardia or complete AVB with sinus rhythm in 8 cases. The types of PP implanted were VVI in 26 cases, VVIR in 2 cases,VDD and DDD in one case.Results All the PP were implanted successfully with good sensing and pacing function.No PP implantation related complications such as wound infection or haematocele, electrode displacement or malfunction were found during follow up period (mean time 26.14±20.25 months).Conclusion The key point of nursing of PP implantation after cardiac valvular replacement is to intensify preoperative psychological nursing, adjust the pre and post operative anti coagulation schemes, and enhance postoperative electrocardiographic monitoring, health promotion and health education, and regular follow up.
