p16与nm23在乳腺癌中的表达及临床意义

摘要 目的 探讨在乳腺癌中p16与nm2 3基因表达以及临床意义。方法 采用免疫组织化学方法对 5 8例乳腺癌和 15例乳腺良性病例中p16与nm2 3基因表达的水平进行研究。结果 乳腺癌中p16与nm2 3的表达水平与良性病变有显著差异 (P <0 .0 5 )。p16在腋淋巴结转移组和发生远处转移组的阳性率低于无腋淋巴结转移组和未发生远处转移组 (P <0 .0 5 ) ,生存期≥ 5年组p16阳性率明显高于生存期 <5年组 (P <0 .0 5 ) ,与生存期呈正相关 ,p16与临床病理类型有关 (P <0 .0 5 ) ;在腋淋巴结阴性组和未发生远处转移组中nm2 3高表达率高于腋淋巴结阳性组和发生远处转移组 (P <0 .0 5 ) ,与生存期呈正相关。p16与患者年龄、肿瘤大小、分期无关 (P <0 .0 5 ) ,nm2 3与临床分期有关 ,但与患者年龄、肿瘤大小、病理类型等方面无显著性差异。p16与nm2 3的表达间无相关性。结论 p16与nm2 3可能与乳腺癌的发生、发展、预后有关。作为两个相互独立的指标 ,对判断肿瘤的生物学行为和预后具有参考意义 。 Objective To investigate the expression and clinical significance of p16 and nm23 gene in human breast cancer. Method The expression of the two protein was investigated by mean of immunohistochemistry in 58 cases of breast cancer.Results The expressions of nm23 and p16 were significantly different between breast cancer and breast benign disease( P <0.05).The positive rates of p16 expression in axillary lymph nodes metastasis group and distant metastasis group were lower than those in patients without axillary lymph nodes metastasis or without distant metastasis( P < 0.05 ).Expression of p16 protein was not correlated with clinical grade?tumor size and life of patients( P <0.05),but tumor pathological type. Expression of nm23 protein product was inversely associated with the axillary lymph nodes involvement and distant metastasis( P < 0.05 ),and was not correlated with tumor pathological type,tumor size and life of patients( P <0.05)but clinical grade. Overall survival was higher in nm23 higher expression cases and p16 positive cases than in the lower positive cases. There was no significant difference between p16 protein expression and nm23 protein expression, but there were higher p16 and nm23 positive reaction in breast cancer with long survival time. Conclusion p16 and nm23 might play an important role in the carcinogenesis,dependent markets to calculate the biological behavior and the prognosis. It is helpful to learn the prognosis by detecting p16 and nm23 together.
