“SARS”患者白细胞和血清免疫抗体检查结果分析 被引量:2

摘要 目的 了解非典型肺炎 (简称“SARS”)患者白细胞总数及其分类计数、血清抗军团菌抗体IgM、肺炎衣原体抗体IgM、腺病毒抗体IgM、肺炎支原体抗体IgM结果及其临床意义。方法 使用全自动血液细胞分析仪进行白细胞总数及其分类计数 ,应用酶联免疫吸附法、胶乳法和斑点金标法进行抗体检测。结果  5 5例“SARS”患者治疗前后白细胞平均值分别为(6 .1 8± 3.6 9)× 1 0 9/L和 (6 .35± 1 .95 )× 1 0 9/L ,无显著性差异 (P >0 .0 5 ) ,治疗前后淋巴细胞平均值分别为 (1 .2 8± 0 .6 3)×1 0 9/L和 (1 .80± 0 .95 )× 1 0 9/L(P <0 .0 5 ) ,治疗前后粒细胞平均值分别为 (4 .4 6± 3.4 8)× 1 0 9/L和 (2 .84± 2 .33)× 1 0 9/L(P <0 .0 5 ) ;5 5例“SARS”患者血清免疫抗体检查结果 :其中 2例患者抗军团菌抗体IgM阳性 ,占 3.6 3% ;4例患者肺炎衣原体IgM阳性 ,占 7.2 7% ,其中 1例合并肺炎支原体抗体IgM阳性 ;1 9例患者肺炎支原体抗体IgM阳性 ,占 34.5 % ,平均滴度是 1∶2 90 .2 1 ,腺病毒IgM全部阴性。结论 本次流行的“SARS”患者并非以上 4种病原体所致 ,只是部分患者合并感染了肺炎衣原体、支原体或军团菌等。“SARS”患者治疗前后白细胞总数无显著性差异 。 Objective To study the changes of the levels of antibodies including anti-legionella IgM?anti-Cp(chlamydia pneumoniae)IgM?anti-MP(mycoplasma pneumoniae)IgM,and the changes in the number and classification of leucocyte in patients with severe acute respiratory syndrome(SARS) Methods Automatic blood cell analysing system,enzyme-linked immuno-sorbent assay(ELISA),immuno-emulsion assay and immuno-colloidal gold assay were employed.Results After treatment,the average number of leucocyte in patients with SARS was(6.35±1.95)×10 9/L versus(6.18±3.69)×10 9/L before treatment(P>0.05);the average number of lymphocyte was(1.80±0.95)×10 9/L versus(1.28±0.63)×10 9/L(P<0.05);the average number of granulocyte was(2.84±2.33)×10 9/L versus(4.46±3.48)×10 9/L(P<0.05). Among 55 patients,two patients(3.63%) were with positive expression of anti-legionella IgM,four patients(7.27%)with positive expression of anti-Cp IgM,nineteen patients(34.5%)with positive expression of anti-MP IgM and the average titer was 1:290.21; one patient with positive expression of both anti-Cp IgM and anti MP IgM; while all the patients with negative expression of anti-adenovirus IgM.Conclusion Legionella,chlamydia pneumoniae,adenovirus and mycoplasma pneumoniae were not the pathogen which caused the epidemic SARS at present, but some patients were co-infected with legionella?chlamydia pneumoniae and mycoplasma pneumoniae.There was no significant change in the average number of leucocyte in patients,but the average number of lymphocyte was significantly increased,while that of granulocyte was significantly decreased.
