摘要
目的通过经颅磁刺激(transcranialmagneticstimulation,TMS)脑性瘫痪(cerebralpalsy,CP)患儿,对早期脑损伤后中枢运动神经传导通路重构机制进行初步探讨。方法对17例3~12岁CP患儿行TMS获得上下肢同、对侧运动诱发电位,并以20例健康者为正常对照。结果CP同侧反应出现率与对照组差异有显著性意义(88.2%vs8%,χ2=3.376,P<0.05),尤下肢更易诱发,重度CP同侧反应肢数≥2条。潜伏期相似型同侧反应主要见于早期脑损伤性CP。结论同侧反应与CP残障度相关,在某种程度上经颅磁刺激诱发同侧反应对判断CP严重度可能具有一定临床意义。
Aim To explore the mechanism of reorgani zation of con-ducting pathways of central motor nerve by transcranial magnetic stimul ation(TMS )in children with cerebral palsy(CP).Methods Seventeen pediatric CP patients,13boys and 4girls,with an average age of 6.5years(3-12)underwent TMS to record the ipsilate ral and contralateral evoked potent ials(EP)of upper and lower limbs.Twenty heal thy children with an average age of 6.9years(3.5-14)were used as controls.Results The occurrence rate of ipsilateral response in CP patien ts was 88.2,especially the patients with severe CP with response in,a rate sig nificantly higher than that in the healthy controls(8%,χ 2 =3.376,P<0.05).Ipsilateral responses were induced in≥2limbs in the severe CP patients.Ipsilateral responses with latencies similar to those of the con tralateral responses were seen chie fly in brain lesions at early stage.Conclusion The ipsilateral responses are cor-related with the disability degree o f CP.To a certain degree,the ipsilateral responses induced by TMS are of considerable clinical significance.
