摘要
目的 :探讨采用阴囊中隔带血管蒂皮瓣尿道下裂一期修复术的方法及疗效。方法 :采用阴囊中隔带血管蒂皮瓣一期修复尿道下裂。Ⅴ型切除阴茎头尿道沟部分海棉体组织 ,成形尿道外口 ,仅留置导尿管引流尿液 ,滚动式挤压排出尿道内分泌物。结果 :除 1例尿道外口狭窄 ,3例尿瘘外 ,其它均排尿通畅 ,且经 1～ 5年随访 ,无尿石形成及毛发生长。结论 :该术式成功率较高。减少了尿道外口狭窄的可能 ,较适合阴茎阴囊型尿道下裂患者。
Objective: To explore methods of one stage urethroplasty with midline scrotal flap with vessel pedicle for hypospadias.Methods: Of one stage,we used urethroplasty with midline scrotal flap with vessel pedicle for treating hypospadias. V-shaped incision was made to cut off some sponge tissue of urethra gulf in penis head for urethroplasty.Urethral cathether was only maintained for urine drainge. And urethral discharges was got off by rolling urethra. Results: This technique was used for 47 cases of hyposedadias of peno-scrotal junction types. Except one case of stricture of metal and three cases of urethral fistula, all the others urinated easily and smoothly.When followed up for 1～5 years, no one had urolithiasis and hair in urethra.Conclusion:This technique has higher success rate. It can reduce the rate of strictuce of metal and is better indicated for hyposedadias of peno-scrotal junction types.
