应用切割球囊行经皮冠状动脉血管成形术时血小板衍生生长因子与6-酮-前列腺素F_(1α)变化的临床意义

摘要 研究应用切割球囊经皮冠状动脉成形术 (CBA)前后血小板衍生生长因子 (PDGF)与 6 酮 前列腺素F1α(6 酮 PGF1α)活性变化的临床意义。将 5 7例患者随机分为切割球囊 (CBA)组 (n =31 )和普通球囊 (POBA)组 (n =2 6 ) ,由冠状静脉窦采血 ,采用生物活性法测定PDGF ,采用酶联免疫标测法(ELISA)检测 6 酮 PGF1α浓度。CBA组与POBA组比较 ,PDGF及 6 酮 PGF1α值在PTCA术后均存在显著性差异 (P <0 0 5 ) ,且PDGF呈时间依赖性上升 ,30min仍维持在高水平。 6 酮 PGF1a值术后两组均呈一过性降低 ,CBA组术后 1 0min恢复至术前水平 ,而POBA组恢复时间延长至术后 30min。提示切割球囊可减轻血管壁损伤 ,降低血小板活化水平 ,可能是降低再狭窄的重要原因之一。 To study the clinical significance of the changes in PDGF and 6 keto PGF 1α in patients before and after cutting balloon angioplasty (CBA) and plain conventional balloon angioplasty (POBA). Blood was obtained from the coronary sinus. The levels of PDGF were determined by bioassay, and the levels of 6 keto PGF 1α were determined by ELISA. The results showed that the levels of PDGF and 6 keto PGF 1α were significantly different after PTCA between CBA group and POBA group ( P <0 05). The level of PDGF was higher after PTCA than that before PTCA, with persistent level at 30 minutes. The level of 6 keto PGF 1α was lower after PTCA than that before PTCA, rising to the normal at 10 minutes in CAB group, but at 30 minutes in POBA group. These results suggested that the cutting balloon could be a new device for coronary angioplasty, which might be beneficial in minimizing the injury to arterial wall and, reducing activation of platelets, comprising one of the factors in of reducing subsequent restenosis.
