摘要
目的探讨小儿早期脑性瘫痪(CP)与宫内感染犤弓形虫(toxoplasmosis)、其他、风疹病毒(rubella)、巨细胞病毒(cytomegalicinclu-sion-virus)、单纯疱疹病毒(hexpes),TORCH犦的关系。方法对170例6个月以内早期CP患儿进行TORCH_IgM检测,并将其分为阴性及阳性两组进行高危因素、并发症、头颅CT、脑干听觉诱发电位(BAEP)、头颅B超、脑电图(EEG)等进行了对比研究。结果阳性45例(26.5%),阳性中以巨细胞病毒为最常见,其次为弓形体;单项感染32例,混合感染13例。高危因素中,阳性组以窒息为多见,其次为早产、异常分娩、颅内出血,阴性组窒息与早产发生率相等,并发症:阳性组比阴性组多,特别是智力低下(MR)及听觉障碍为最明显。阳性组的头颅CT、BAEP、头颅B超、EEG异常率比阴性组高,尤以头颅B超为明显,阳性组治疗后在短期内转阴率为91%。结论宫内感染是CP患儿中枢神经损伤、病理损害的重要原因。
Aim To investigate relationship between cerebral palsy(CP)and intrauterine infectiontoxoplasmosis,rubella,cytomegal ic inclusion-virus,hexpes(TORCH)of pediatrics.Methods TORCH_IgM examination were carried out in 170cases of CP babies within 6months and classed them into positive and negative groups.High r isk factor,complication,skull CT,BAEP,B-ultrasound and EEG were examined a nd compared in the two groups.Results There were 45cases(26.5%)positive,32cases of singlarinfection and 13cases of compound in fection.Among them,cytomegalovir us infection is the most common ones and toxoplasma is next to it.In risk factor research,chok is the most common ones,early born,abnormal labor,inter-cranial hemaphagia is next to it.Whi le in the negative group,cholk is equiva-lent to early born.Complications:c omplications in positive group is greater than that of the negative group,especially Mental retadation and auditi on disorder is the most obvious cause.A bnormal rate of head CT,BAEP ,B-ultrasound,EEG is higher than that of the negative group,especially i n B-ultrasound.After treatment,91%of the items in positive group turned into negative.Conclusion Intrauterine infection is a most imp ortant cause of CP in pediatrics.TORCH infection plays a n important role in central nerve system injury and pathological damage.
