摘要
目的 观察前列腺素E1(PGE1)对肾综合征出血热少尿期患者肾血流和血浆内皮素(ET)的影响及临床疗效。方法 将重型、危重型少尿期患者随机分为对照组 ( 4 4例 )和治疗组 ( 4 2例 )。对照组给与一般治疗 ,治疗组以脂质体包裹PGE110 μg +10ml生理盐水静脉注射 ,每日 1次 ,疗程 7天。观察病程变化 ,彩色多普勒测量双侧肾动脉血流 ,放射免疫法测定血浆内皮素水平。结果 治疗组少尿期明显缩短 (P <0 .0 5 ) ,血浆ET水平降低 (P <0 .0 5 )。治疗组治疗后双肾动脉内径 (D)、收缩期峰值血流速度 (Vmax)、舒张末期血流速度 (Vmin)、平均血流速度 (Vmean)、时间速度积分 (VTI)较治疗前明显增加 (P <0 .0 5 ) ,肾动脉血流量 (Q)增加 (P <0 .0 1)、阻力指数 (RI)下降 (P <0 .0 1)。结论 脂质体包裹PGE1可增加重型、危重型肾综合征出血热少尿期患者肾血流量 ,降低血浆ET水平 ,缩短少尿期 ,促进肾功能恢复。
Objective To explore effects of prostaglandin E 1 on renal blood flow and plasma ET in kidney failure caused by hemorrhagic feve r with renal syndrome.Methods The patients were randomly divided into two groups,expe rimental group(42 cases)who received liposome coating PEG1 and control group(44 cases)who received conventional treatment.The renal arterial blood flow before a nd after treatment was examined by color Doppler ultrasound in both groups.Plas ma ET was detected by radioimmunity assay.Results In the experimental group,the variables of renal arteri al blood flow including the diameter of renal artery(D),velocity of systole peak flow(Vmax),velocity of diastole flow(Vmin),mean flow velocity(Vmean),blood flow of renal artery(Q),velocity time integral(VTI) and drag index(RI) significantly increased after treatment;oliguria stage shortened with plasma ET level decreas ing simultaneously.Conclusion The application of liposomes coating PGE1 in treatme nt of HFRS with renal failure is beneficial to shorten oliguria phase by increas ing renal blood flow and decreasing plasma ET.
