大鼠肾结石钠/二羧基转运蛋白-1的表达变化及碳酸氢钾的干预作用 被引量:1

摘要 目的 研究肾组织钠 /二羧基转运蛋白 (SDCT1)与尿枸橼酸变化的关系及其在肾结石形成中的作用 ,并观察碳酸氢钾对SDCT1表达的影响 ,为制定肾结石防治措施提供新的思路。方法 雄性Wistar大鼠分为对照组、致石剂组及碳酸氢钾干预组。采用Northernblot检测大鼠肾组织SDCT1mRNA水平的改变 ,免疫组化方法观察SDCT1在肾组织的分布及表达变化。常规病理观察肾结石形成情况。常规生化方法测定血、尿枸橼酸等生化指标。结果 致石剂组大鼠肾组织SDCT1mRNA水平显著高于对照组 ;SDCT1分布于近端肾小管刷状缘 ,其表达量显著高于对照组 ,尤以扩张的近端肾小管为甚 ;尿枸橼酸水平及尿pH值显著低于对照组 ;肾结石形成率为 10 0 %。碳酸氢钾SDCT1mRNA水平显著高于致石组 ;SDCT1表达也显著上调 ;尿枸橼酸水平及尿pH值显著高于致石剂组 ;肾小管间质等病变明显减轻 ;肾结石形成率为 3 0 % ,显著低于致石剂组。结论 肾组织SDCT1表达上调可能是低枸橼酸尿的重要原因 ,与肾结石的形成有密切关系。碳酸氢钾可下调肾结石大鼠肾组织SDCT1的表达 。 Objective To investigate the change of Na +/dicarboxylate co transporter expression in rat nephrolithiasis induced by lithogenic agent and the preventive effect of potassium bicarbonate. Methods Twenty eight male Wistar rats were divided into 3 groups: control, lithogenic agent group and potassium bicarbonate group. The expression of SDCT1 mRNA in the kidney was determined by Northern blot analysis. The change of SDCT1 protein abundance was measured by immunohistochemical staining with anti hNaDC 1 antibody. The plasma and urinary biochemical parameters, such as citrate, oxalate, uric acid and calcium were analyzed by routine biochemical method. The stone formation in renal tissue was examined and the pathological change of kidney was observed using routine pathological technique. Results SDCT1 mRNA was expressed in the normal renal tissue and SDCT1 was found in apical membrane of renal proximal tubule. Compared with control group, lithogenic agent group had a higher expression of SDCT1 mRNA and protein abundance in the kidney, but a lower level of citrate in the urine. However, the citrate administration group showed a higher level of SDCT1 mRNA and protein abundance. The urinary citrate levels increased and the pathologic changes such as tubular dilation and inflammatory cells infiltration were alleviated. Conclusion The upregulation of hNaDC 1 mRNA and protein abundance in the kidney has a close relationship with hypocitraturia, which may play an important role in pathogenesis of nephroliathisis. Potassium bicarbonate treatment has a beneficial effect on rat with nephrolithiasis induced by lithogenic agent.
