摘要
目的 :总结临床应用微创技术治疗长节段粉碎性胫腓骨骨折的经验。方法 :1995年 10月~ 2 0 0 0年 10月应用跟骨牵引复位 ,夹板外固定术治疗该类骨折 19例 ,应用骨外固定支架固定术治疗该类骨折 2 7例 ,应用自制桥接钢板内固定术治疗该类骨折 12例。结果 :5 8例术后X线检查显示骨折力线正常 ,经 1~ 2年随访 ,5 0例如期骨性愈合 ,5例延迟愈合 ,3例骨不连经再次手术而治愈。结论 :(1)微创技术贯穿于治疗过程的始终 ,而不是单纯采用什么微创术式。 (2 )微创术式的技术核心是大体恢复骨折的力线 ,相对稳定骨折端 ,尽可能减少骨折块血运的再破坏。 (3 )根据临床的实际情况 ,合理选择微创固定系统和操作技巧是成败的关键。
Objective:To summarize the clinical experience oftreatment for the long-segment comminuted fracture of tibia and fibula with the micro-traumatic-technique.Methods:Fifty-eight cases of long-segment comminuted fracture of tibia and fibula had been treated with the micro-tramatic-technique from Nov.1995 to Nov.2000,which included 19 cases of skeletal traction through calcaneus combined with external splintage,27 cases of external fixation of fracture, and 12 cases of internal fixation using self-made bridage-shaped steel plates.Results:All the patients had been following-up surveyed for 1~2 years post-operatively,and the X-ray examination showed that the fracture line were good reduction in early stage after operation,fifty cases healing well and 5 cases delayed union,and 3 cases had secondary operation because of nonunion.Conclusions:It is very important to select the reasonable methods to treat the long-segment comminuted fracture of tibia and fibula according to the clinical situation.
