摘要
用经颅多普勒(TED)超声诊断仪观察了25例临床脑死亡患者大脑中动脉血流速度(FVmca)的变化。结果发现,25例FVmca均有相似的改变:首先,FVmca频谱中舒张期末血流速度(Vd)明显降低,脉搏指数(PI)增加;继之,Vd继续下降;最后,出现Vd反向,所有患者均显示收缩期正向流速和舒张期反向流速的特征性双向TCD波形。持续6h后,出现脑死亡征象。提示TCD可作为一项新的无创伤性判断脑死亡的标准。
A transcranial Doppler (TCD) ultrasonic diagnostic apparatus was used to measure the flow velocities of blood in the middle cerebral artery (FVmca)in 25 clinical brain death patients. The results showed that there was a similar change of FVmca in 25 patients. Firstly; the end-diastolic blood velocity (Vd) decreased remarkably in the FVmca spectrum; the pulsatility index (PI) measurements increased. Secondly, Vd continued to decrease. Finally, all patients showed a characteristic to-and-fro TCD pattern with antegrade flow in systole and retrograde flow in diastole. Patients showed sign of brain death 6 hrs later. TCD was found to be a useful, new and non-invasive criterion in the determination of brain death.
