喷砂制洞和复合树脂充填对邻牙合面洞细菌微渗漏影响的实验研究 被引量:4

摘要 目的 :通过离体牙细菌微渗漏实验 ,评估喷砂制洞、5种树脂类材料和银汞合金充填对邻牙合面洞边缘封闭质量的影响。方法 :将 48个完整无龋的离体前磨牙及第三磨牙随机分为两个实验组 ,第一组采用常规涡轮机裂钻制备窝洞 ,第二组采用喷砂制洞。所有实验样本制备邻牙合面洞模型 ,近中牙合面洞的颈缘位于釉质区 ,远中牙合面洞的颈缘位于根部牙骨质区。用树脂材料充填窝洞 ,在 37℃厌氧环境下浸泡于细菌悬液中2 0h。然后通过组织学检查评估细菌渗漏情况。结果 :所有实验组与对照组银汞合金之间存在显著差异(P <0 .0 1 )。CLEARFILAP -X/SEBOND的边缘封闭质量与DURAFILLVS、P - 50的差异最显著 (P <0 .0 1 ) ;喷砂制洞与传统切割制洞对于细菌渗漏情况的影响无明显差异 (P >0 .0 5) ;近 (远 )中壁微渗漏较颈壁严重 (P <0 .0 5)。结论 :树脂材料比银汞合金具有更好的边缘封闭效果 ,更适合于后牙的充填修复 ; AIM:To evaluate the quality of the air a brsaion pretreatment and marginal seals of 5 resin composites on microleakage of mesio (disto)-occlusal restorations.METHODS:Fresh extracted 4 8 intact premolars and third molars were divided randomly into two groups. In th e first experimental group, conventional mesio (disto)-occlusal cavities were pr epared with fissure burs. In the second group, the same cavities were prepared w ith air abrasion.Then the cavities were reconstructed with different filling mat erials.The quality of the marginal seals was evaluated by submerging the teeth i n a bacterial suspension and incubating in an anaerobic milieu at 37℃ for 20 ho urs. The teeth were subsequently examined histologically and bacteriologically. RESULTS: Nonparametrical statistical analysis (Kruskal-walli sfa) revealed highly statistical significant differences in the penetration of b acteria between the amalgam control samples and all of the tested samples(P< 0.01).Difference between CLEARFIL AP-X/SE BOND and DURAFILL VS&P-50 were the mos t (P<0.01).One-way ANOVA (wilcoxon) de- monstrated no significant differe nce in microleakage between the fissure bur group and the air abrasion group ( P>0.05). Microleakage in the mesial/distal walls of Class Ⅱ restorations was much severe than that in the gingival walls(P<0.05).CONCLUSION:A comparison of the restorative materials on the respect of bacterial mi croleakage suggested that there is a significantly higher quality of the margina l sealing in mesio (disto)-occlusal cavities filling with resin composites than the amalgam. Cavities prepared with air abrasion showed no significant less micr oleakage than the diamond burs.
