胆管癌组织中凋亡基因Fas、FasL的表达及其意义 被引量:1

The expression and significance of Fas,FasL in extrahepatic cholangiocarcinoma

摘要 目的 :探讨胆管癌组织中凋亡基因Fas、FasL的蛋白表达及其与胆管癌的发生、发展及预后判断的关系。方法 :应用免疫组化S P法检测 2 2例胆管癌组织及 10例先天性胆管囊肿组织中Fas、FasL的表达。结果 :胆管癌Fas蛋白表达率为 4 5 .4 5 % (10 / 2 2 ) ,显著低于对照组 90 % (9/ 10 ) (P <0 .0 5 ) ;而FasL蛋白表达率为72 .73% (16 / 2 2 ) ,显著高于对照组 (P <0 .0 5 )。淋巴结或远处转移的患者中Fas表达率明显下降 (P <0 .0 5 ) ;FasL阳性表达主要集中在TNM分期Ⅲ~Ⅳ期、有淋巴结或远处转移的患者中 (P <0 .0 5 )。Fas、FasL表达无显著相关性。FasL阳性而Fas阴性的患者累积生存率高于其他患者 (P <0 .0 5 )。结论 :胆管癌细胞中存在Fas表达下调而FasL表达上调现象 ,并与临床分期及是否转移密切相关。Fas。 Objective:To investigate correlation between the expression of gene proteins Fas,FasL in exhepatic bile duct cancer and the development of the carcinoma.Methods:Immunohistochemical S P method was used to examine the expression of Fas,FasL protein in 22 cases of cholangiocarcinoma and 10 cases of bile duct cyst.Results:The positive rate of Fas(45.45%) in cholangiocarcinoma decreased significantly(P<0.05),while that of FasL(72.73%) increased remarkably(P<0.05). The positive rate of Fas was lower in the low differentiation and metastasis groups(P<0.05). The positive FasL expression was inversely proportional to the stage Ⅲ~Ⅳ and metastasis of the carcinoma. The survival rate was significantly higher in patients with FasL+Fas than that of the rest(P<0.01).Conclusion:The down regulation of Fas and up regulation of FasL in cholangiocarcinoma indicates that the imbalance of Fas/FasL system might play a role in the escape of tumor cells from immune surveillance and predicts the malignant behavior of the carcinoma. The detection of Fas,FasL may be useful in assessing the development of the cancer and judging prognosis.
