先天性主动脉瓣、瓣上及瓣下狭窄的影像学诊断 被引量:3

Imaging Diagnosis of the Congenital Aortic Stenosis、 Subvalvular Aortic Stenosis and Supravalvular Aortic Stenosis

摘要 目的 :提高影像学诊断先天性主动脉瓣、瓣上及瓣下狭窄的水平。材料和方法 :主动脉瓣狭窄 12例 ,主动脉瓣下狭窄 5例 ,主动脉瓣上狭窄 7例 ,均行X线检查 ,MRI 2 1例 ,心血管造影 (CAG) 15例 ,手术证实 15例。结果 :主动脉瓣狭窄MRI显示瓣膜增厚、瓣口狭窄 2例 ;主动脉瓣下隔膜型狭窄MRI示 3例 ,CAG示 1例 ;MRI示主动脉瓣上纤维嵴型狭窄 6例及发育不良型狭窄 1例。结论 :X线平片难以提供确定诊断的依据 ;超声心动图对主动脉瓣上狭窄的诊断有限 ;除主动脉瓣狭窄外 ,MRI可弥补超声心动图诊断中的不足 ,二者结合可达到术前诊断的目的 ,除特殊适应症外 ,无须行CAG检查。 Purpose:To assess the diagnostic efficacy of imaging in the congenital aortic stenosis,subvalvular aortic stenosis and supravalvular aortic stenosis. Materials and Methods:12 cases had the aortic stenosis,5 cases had the subvalvular aortic stenosis,and 7 cases had the supravalvular aortic stenosis. All cases were examined by X ray,21 of 24 cases had MRI,15 cases had cardiovascular angiography (CAG) and 15 cases were confirmed by operation. Results:The thinkened aortic valve and the valvular orifice stenosis were seen in 2 cases with the aortic stenosis on MR. The membranous type stenosis were seen in 3 cases with subvalvular aortic stenosis on MR and in 1 case on CAG. All cases with supravalvular aortic stenosis were examined by MRI,6 of 7 cases showed fibromuscular type stenosis and 1 of them showed dysplasia type stenosis on MR. Conclusion:X ray examination is difficult in supplying the definite diagnosis; Echocardiography had limited value in diagnosing the supravalvular aortic stenosis; MRI could remedy the weakness of echocardiography.Combining MRI with echocardiography,it is able to attain the diagnosis before operation. Except for the special indication,the examination of CAG was not needed.
