摘要
目的 探讨端粒酶活性检测及与细胞学联合应用对良恶性胸腔积液鉴别诊断的价值。方法 用 TRAP-杂交分析法检测了 76例患者胸腔积液 (包括 4 4例良性胸腔积液和 32例恶性胸腔积液 )端粒酶活性 ,并与常规细胞学方法进行比较。结果 端粒酶活性在恶性胸腔积液中阳性率为 2 7/32 (85 % ) ,良性胸腔积液中阳性率为 11/44 (2 5 % )。端粒酶活性检测诊断恶性胸腔积液的敏感性为 0 .85 ,特异性 0 .75 ;和细胞学联合诊断恶性胸腔积液的敏感性 0 .91,特异性 0 .75。结论 端粒酶活性检测对恶性胸腔积液的诊断与细胞学方法比较其敏感性较高而特异性较低。
Objective To Investigate the differential diagnostic value of telomerase activity detection alone or combined with cytology for benign and malignant pleural effusions. Methods Seventy six specimens of pleural effusions, including 44 nonmalignant pleural effusions and 32 malignant pleural effusions, were examined for telomerase activity and cytology by telomere repeat amplification protocol hybridization analysis and hematoxylin eosin staining, respectively. Results Twenty seven of 32 (85 %) samples with malignant pleural effusions showed positive telomerase activity and 11 of 44 (25 %) samples with nonmalignant pleural effusions had weak enzymatic activity. The sensitivity of telomerase activity for the diagnosis of malignant pleural effusions was 0.85 and specificity was 0.75. The diagnostic sensitivity and specificity of combined use of telomerase activity detection and cytology were 0.91 and 0.75 respectively. Conclusion Compared with cytology, telomerase activity possesses higher sensitivity and a little lower specificity for the diagnosis of pleural effusions. The combined use of telomerase activity detection with cytology indicates much more advantage than that of any single method for the differential diagnosis of malignant pleural effusions from benign pleural effusions.
