海上氦氧150m饱和-182m巡回潜水潜水员肺功能的变化 被引量:6

Changes of divers′ pulmonary functions in 150 m heliox saturation-182 m excursion diving in the open sea

摘要 目的 探讨海上大深度氦氧饱和潜水对人体肺功能的影响。方法  8名潜水员 ,进行海上氦氧 15 0 m饱和 - 182 m巡回潜水 ,测定潜水员进舱前、加压、饱和暴露、出舱后立即和出舱后 2 4 h的肺通气功能。结果 高气压暴露阶段的肺功能变化显示 ,除 FVC在各阶段的值差异无显著性外 ,其他各指标与深度呈负相关。 FEV1 .0 、FEV1 % 在 15 0 m饱和停留阶段与基础值相比有显著性降低 (P<0 .0 5 ,<0 .0 1) ;MMF在 70 ,110 ,15 0 m饱和停留阶段与基础值比较有显著性降低 (P<0 .0 5 ,<0 .0 1,<0 .0 1) ;PEFR在加压 110 m和 15 0 m饱和停留阶段与基础值和 30 m测定值相比有显著性降低 (P<0 .0 1,<0 .0 5 ,<0 .0 1) ;在 15 0 m饱和停留阶段 ,FEF2 5% 与基础值、30 m测定值比较都显著降低 (P <0 .0 1,<0 .0 5 ) ;FEF50 % 与基础值、30 ,70和 110 m测定值比较都显著降低 (P<0 .0 1,<0 .0 5 ,<0 .0 5 ,<0 .0 5 ) ;FEF75% 与基础值、30和 70 m测定值比较都显著降低 (P<0 .0 1,<0 .0 5 ,<0 .0 5 )。在 15 0 m饱和停留阶段 ,FEF2 5% 、FEF50 % 、FEF75% 较基础值减低的幅度分别为 2 5 .5 1% ,4 2 .35 % ,5 6 .84 % ,表现出肺容量越低对应的流量变化越明显。出舱后的肺功能参数均在正常范围内波动。出舱后立即测得的 MMF较基? Objective To determine the changes of divers′ pulmonary functions in response to the prolonged hyperbaric exposure in saturation diving.Methods 8 male navy divers took part in a single 150 m heliox saturation 182 m excursion diving in the South China Sea.Dynamic pulmonary functions were measured before diving,during compressing phase,under saturation depth,immediately after surfacing,and 24 h later.Results Under pressure,except FVC,all the other indexes were negatively correlated with depth.FEV 1.0 and FEV 1% decreased significantly at 150 m saturation depth,compared with the basic value( P <0 05 and P <0 01).Compared with the base value,MMF tested at the compressing 70 m stop,110 m stop and the 150 m saturation depth was significant decreased( P <0 05, P <0 01 and P <0 01).PEFR at 110 m and at 150 m saturation stages was significantly lower than that at base( P <0 01,<0 01)and that at 30 m( P <0 05, P <0 01);During 150 m saturation stage,FEF 25% was significantly decreased,compared with that at base and 30 m( P <0 01,<0 05);FEF 50% was significantly decreased,compared with that at base,30 m,70 m and 110 m( P <0 01,<0 05,<0 05,<0 05);FEF 75% was significantly decreased,compared with that at base,30 m,70 m( P <0 01,<0 05,<0 05).Compared with the base value,FEF 25% ,FEF 50% and FEF 75% were decreased by 25.51%,42.35%,and 56.84% respectively,which suggested that,under pressure,the lower was the pulmonary volume,the more significant was the decrease of flow.After surfacing,all data were within normal range,but a significant decline was seen in MMF and FEF 50% ( P <0 05).24 h later,FEF 25% was restored.Conclusions Dynamic pulmonary functions are mainly affected by the high density gas,and presents small airway dysfunction.But these effects are functional and reversible.The manipulation of the experimental conditions in this saturation diving is safe and successful.
