摘要
目的 探讨自动调节持续气道正压通气 (Auto CPAP)治疗阻塞性睡眠呼吸暂停低通气综合征 (OS AHS)的方法和疗效。 方法 经多导睡眠图 (PSG)检查确诊的中、重度OSAHS 2 8例 ,采用Auto CPAP呼吸机全夜不少于 7h的治疗 ,并同步进行血氧饱和度 (SaO2 % )监测。 结果 经Auto CPAP治疗以后 ,鼾声、白天嗜睡、夜间多尿有非常明显的改善。经PSG检查 ,睡眠呼吸暂停指数 (AHI)由治疗前的 5 8.4± 18.4下降到 10 .2± 6 .1,夜间最低血氧饱和度 (LSaO2 % )由治疗前的 6 0 .5± 16 .8上升到 87.1± 3.8,全夜小于 90 %血氧饱和度时间由治疗前的 16 3.8± 112 .5min减少到 9.8± 14 .9min。治疗前后对比差异均有显著性 (P <0 .0 0 1)。本组OSAHS者用Auto CPAP治疗的 95 %可信压力为 11.2± 2 .0cmH2 O ,平均鼻面罩漏气为 0 .2± 0 .1L/s。 结论 Auto CPAP是治疗OSAHS一种有效方法 ,能使临床症状明显改善 ,但在治疗中须注意鼻面罩的合适与舒服。
Objective To explore the effect of auto continuous positive airway pressure (Auto CPAP) on obstructive sleep apnea hypopnea syndrome (OSAHS). Methods Twenty eight patients with moderate or serious OSAHS diagnosed by means of polysomnogram (PSG) were treated by Auto CPAP at least 7 hours during the night. Meantime, the oxygen saturation in arterial blood (SaO 2) was monitored as well. Results After Auto CPAP treatment, the clinic symptoms were reduced, especially obvious in the case of heavy snore, daytime somnolence and frequent nighttime micturition; apnea plus hypopnea index (AHI) was decreased from 58.4±18.4 to 10.2±6.0; the lowest SaO 2 in the night was increased from 60.5%±16.8% to 87.1%±3.8%; the time showing SaO 2 less than 90% in the night was decreased from 163.8±112.5 min to 9.8±14.9 min(P< 0.001 ).The reliable pressure of 95% OSAHS patients treated by Auto CPAP was 11.2±2.0 mmH 2O,the average air leakage was 0.2±0.1 L/s. Conclusion Auto CPAP is an effective way in treating OSAHA, it can reduces the clinic symptoms remarkably. However, like the traditional CPAP treatment, the convenience and suitability of the mask are important.
