摘要
目的 探讨不同植片的光学性角膜移植术的远期屈光变化。方法 对圆锥角膜 ,病毒性角膜炎 ,外伤性、霉菌性等所致的角膜白斑 ,角膜营养不良采用不同大小的角膜植片行穿透角膜移植手术 ;术后随访 10个月至 8年 ,平均 48个月。结果 角膜植片≤ 7mm与 >7mm的角膜植片的两组术后散光发生率以小植片组为高 ,两组比较有显著性差异(P <0 0 5 )。术后角膜散光轻微不影响视力者 2 4眼 (3 0 8% ) ,散光明显者 5 4眼 (69 2 % ) ,其中不规则散光者 8眼(14 8% ) ,顺规则散光者 2 5眼 (4 6 3 % ) ,逆规则散光者 2 1眼 (3 8 9% )。结论 角膜植片、植床大小的最佳配置 ,光学中心的良好对合 ,较好的缝合技术 ,术后角膜地形图指导下的选择性拆线等方法有助于减少手术源性的屈光不正 。
ObjectiveTo investigate the refraction change at a specified future date in optics cornea transplant with diameter cornea graft. MethodsOptic cornea transplant were performed on 76 cases (78 eyes ) with leukoma and dystrophia caused by keratoconus,viral keratitis, traumatic and mycotic keratitis. Follow up period was 10 month~8 year.ResultsThe incidence of astigmatism was significantly higher in the group cornea graft ≤7 mm when compared to >7 mm group( P <0 05).Postoperative cornea astigmatism of 24 eyes (30 8%) did not affect vision,severe astigmatism 54 eye(69 2%) and irregular astigmatism 8 eye(14 8%),with rule direct astigmatism 25 eye(46 3%)and astigmatism against the rule 21 eye(38 9%).ConclusionThe suitable cornea graft, matched optical center, good skill of sewing and removing the suture are helpful for reducing astigmatism after cornea transplantation.
