摘要
目的 探讨实时三维超声心动图的临床应用价值。方法 应用实时三维成像技术对 37例正常人心脏与各种心脏病变患者进行实时三维成像 ,其中先天性心脏病 1 7例 ,房间隔缺损修补术后 1例 ,室间隔缺损修补术后 2例 ,风湿性二尖瓣狭窄 1 3例 ,高血压心脏病 7例 ,冠心病 8例 ,冠脉搭桥术后 1例 ,肥厚性心肌病 2例 ,二尖瓣人工瓣置换术 5例 ,二尖瓣脱垂 1例 ,感染性心内膜炎 1例 ,糖尿病心肌病变 1例 ,心包积液 1例 ,膀胱肿瘤 3例 ,胎儿 1例。另通过室壁瘤模型、动脉粥样斑块模型以及离体猪心的三维检查 ,验证实时三维成像的准确性。结果 ①临床应用结果证明实时三维成像能显示扫描区房室壁的整体形态 ;②可显示正常二尖瓣、三尖瓣的完整形态及活动 ,显示瓣膜病变的整体范围 ;③能完整地显示房间隔、室间隔缺损的立体形态 ;④可观察室间隔缺损修补术后补片的整体形态及空间方位 ;⑤显示法洛四联症等病变复杂的空间结构 ;⑥清楚显示人工瓣的整体形态与活动 ;⑦三维图像与实验模型一致。结论 实时三维超声心动图能实时显示扫描区心脏结构的立体形态 ,同传统的二维超声心动图相比 ,其能提供更多三维解剖结构与功能信息 。
Objective To explore the initial practice o f real time three-dimensional echocardiography (RT-3DE). Methods One hundred and one cases (54 male, 47 female, mean aged 41.6 years) were examined by RT-3DE, including 37 normals, 17 patients with congenital diseases, 1 patient with artial septal defect repaired, 2 patients with ventricular septal defect repaired, 13 patients with rheumatic mitral valve stenosis, 7 patients with hypertension, 8 patients with coronary heart diasease, 1 patient with coronary bypass operation, 2 patients with hypertrophic myopathy, 5 patients with mitral prothetic valve, 1 patient with mitral valve prola pse, 1 patient with infective endocarditis, 1 patient with diabetes mellitus, 1 pa tient with pericardial effusion, 3 patients with tumors of bladder, 1 patient was fetus. Also, some models were made, including left ventricular aneurysm model after acute myocardium infarction, artery atheromatous plaque model, and porcine heart in vitro, and imaged by RT-3DE. The RT-3DE images were compared with models. Results ① The whole views of atrial and ventricular wall in scaned area were dispayed. ② The stereoscopic views and motion of normal mitral and tricuspid valves were showed. ③ The stereoscopic views of atrial and ventricular septal defects were observed. ④ The whole view of the patch in patient with ventricular septal defect repaired were shown. ⑤The complex spatial relations of heart structures in patients with fallot tetralogy were displayed clearly. ⑥The whole views and the motion of prothetic mitral valves were displayed. ⑦ The RT-3DE images of models were consistent with the shape of models. Conclusions The stereoscopic views of the structures and lesions of heart could be displayed in real time by RT-3DE. Compared with two-dimensional echocardiography, RT-3DE could give us more information of structures and function of heart and have great potential value in its clinical utility.
