摘要
目的探讨转移性晚期肺癌个体化综合治疗的疗效及提高生存质量的临床价值。方法对我院1996年1月-2000年6月,收治的78例肺转移性恶性肿瘤进行回顾性分析。全部病例其原发灶均经细胞学或病理学证实,肺转移灶亦经胸部CT和细胞学或病理学证实。全组病例78例,男性47例,女性31,中位年龄65岁(55～78岁)。单个病灶22例,多发转移灶56例。单纯化疗组25例,化疗+放疗组(放化疗组)53例,78例中70例化疗间期服用中药。结果78例肺转移性恶性肿瘤中放化疗组总有效率为(RR)58.4%(31/53),单纯化疗组总有效率为16%(4/25)(P<0.01)。放化疗组中肺癌有效率(RR)50%中位生存期MST10个月;鼻咽癌有效率(RR)64%,MST15个月;乳腺癌有效率(RR)56%,MST18个月;食道癌有效率(RR)58%,MST13个月;治疗前Karnofsky评分均数76.4±7.02;治疗后均数82.31±6.63,(P<0.01)。中药对放化疗有辅助作用。结论本组病例中以鼻咽癌、乳腺癌、肺癌及食道癌肺转移的治疗结果提示对放化疗敏感,配合中药,可有效地提高生活质量具有临床价值。
Objective To investigate the comprehensive tr eatment value on the malignant tumor of lung metastasis.Methods From January,1991to June,2000,our hospital admitted 78cases of malign ant tumor with lung metastasis.All cases were proved by Cytology and Pathology.The metastatic lesions were proved by CT and Cytology and Patholo gy.There were 47male cases and 31female cases in the group.The average age was 65years old(55～78).Single lesion occupied 22case,and multiprimary lesions reached 56cases.Pure chemotherapy were used in 25cases,and 53cases were tre ated with both radiotherapy and chemotherapy;70among 78cases took traditional Chinese drugs at the same time.Results The total effective rate of the comprehensive group(RR)58.4%(31/53).The other group(RR)16%(4/25).The effective rate of lung cancer in metastatic tumor(RR)50%MST was 10month;nasopharyngeal carcinoma (RR)64%MST 15month;breast cancer(RR)56%MST 18month;esophagealcancer(RR)58%MST 13month.KPS:Before treatme nt was 76.4and after treatment was 82.31(P<0.01).Traditional chinese medicine coul d help the radiotherapy and chemotherapy in the treatment.Con-clusions The result shows that all malignant t umor of lung metastasis are sensitiv e to radiotherapy and chemotherapy.There is clinical value by assisted use of tra ditional chinese medicine.[
