摘要
目的 探讨围绝经期、老年期子宫脱垂的原因及防治措施。方法 对 44例围绝经期、老年期子宫脱垂病人进行分析。根据子宫脱垂的程度及能否耐受手术 ,分别给予上子宫托、阴式全子宫切除术 +阴道前后壁修补术、单纯阴道前后壁修补术、Manchester术。结果 本组 38例在非产褥期发病 ,距末次分娩至少 5年以上 ;有 39例接受了相应的手术治疗 ,4例上子宫托 ,1例因近期合并尿瘘给予对症支持疗法。结论 本病发病的主要原因不再是分娩损伤所致 ,而是与卵巢雌激素水平低落、生殖器官萎缩、子宫诸韧带及盆底组织松弛无力有关 ,治疗措施以手术为佳。
Objective To study the causes and therapy of uterine prolapse during peri-menopause and senility.Methods Fourty-four patients suffered from prolapse of uterus in peri-menopause and senility were analyzed.Based on the degrees of uterine prolapse and patient's bearing surgery remedy,the patients were treated individually with uterus tray,vaginal hysterectomy uniting reconstruction of vaginal wall in both front and rear,simple reconstruction of vaginal wall in both front and rear,and Manchester operation.Results Thirty-eight cases suffered the disease beyond puerperium for over 5 years after their last delivery.Thirty-nine cases had experienced relative surgery remedy.Four cases had been laid uterus tray.One case was given auxiliary treatment because of recent urinary fistula.Conclusion The disease doesn't mainly result from delivery injury no longer.It does relate to lower estrogen of ovarian,withered reproductive organs,and slackness of uterine ligaments and pelvic floor.The best method of treating the disease is operation.
